If you-re a Primary Care focused Community Health Nurse (CHN) with acute care experience, this is a rare opportunity to practice at full scope in a supportive, collaborative environment while enjoying one of Canada's most distinctive northern lifestyles.
Work in a city where your 10 minute commute may involve seeing the northern lights dance above the boreal tree line, paddling in Great Slave Lake in twilight evenings and vibrant local farmers markets with cuisine from all over the world.
This position plays an important role in delivering primary care services within an integrated health system in Yellowknife. The Community Health Nurse supports individuals and families through a broad range of services aimed at maintaining and improving overall health. The role requires strong clinical knowledge, sound decision-making, and the ability to work both independently and collaboratively. A consistent focus on culturally safe, respectful, and trauma-informed care is essential in all interactions.
Overall, the CHN helps ensure that care is accessible, coordinated, and responsive to community needs, while contributing to improved health outcomes and system-wide priorities.
Your Role
- Care for patients of all ages as a key member of an Integrated Primary Care Team in Yellowknife.
- Advocate for patients while delivering safe, compassionate, non emergency care.
- Respond confidently to acute concerns and medical crises using NWT nursing standards.
- Build long term relationships through chronic disease management and follow up.
- Keep communities well through proactive preventive screening.
- Coordinate seamless care for paneled patients, from assessment to intervention.
- Practice nursing with an expanded scope of the CHN function.
- Potential to participate in a downtown Nurse-led Clinic pilot program
Key Responsibilities
- Provide comprehensive community health services including health promotion, disease prevention, and management of acute and chronic conditions across the lifespan
- Lead case management and coordinate care for patients, including high-risk and complex cases, within an interdisciplinary team
- Assess, plan, implement, and evaluate patient care, ensuring timely follow-up and preventative screening
- Deliver culturally safe, trauma-informed patient and family education to support health and wellness
To be successful in this role, typically, the above qualifications would be attained by:
The successful completion of a Nursing Degree, and two (2) years of recent nursing experience in an acute care setting or as a Community Health Nurse.
