Regional Cardiac Care Coordinator

About the role

OVERVIEW

The Regional Cardiac Care Coordinator is responsible for coordinating timely access to cardiac services across the continuum of care. The role supports referral triage, booking, wait list management, patient assessment, education, data reporting, and system coordination for patients requiring cardiac catheterization, PCI, cardiac surgery, arrhythmia, and structural heart services.

Working collaboratively with physicians, nursing, allied health, administrative staff, referring hospitals, and provincial partners, the Regional Cardiac Care Coordinator supports safe, equitable, and efficient patient access to cardiac care. The role also contributes to quality improvement, policy and procedure development, accreditation readiness, and alignment with best practice and provincial standards. This version also reflects the core coordination, triage, data, and regional liaison functions described in the uploaded RCC position description.

EDUCATION

  • Baccalaureate Degree in Nursing required

  • Current registration in good standing with the College of Nurses of Ontario required

  • Current BCLS certification required

QUALIFICATIONS

  • Minimum 5 years of recent cardiology experience required
  • Previous Cath Lab experience preferred
  • Previous patient education experience preferred
  • Experience in clinical triage, referral coordination, and wait list management preferred
  • Experience with data collection, reporting, statistical analysis, and quality assurance preferred
  • Computer proficiency required, including experience with word processing, spreadsheets, databases, and electronic clinical systems
  • Excellent clinical assessment and critical thinking skills
  • Strong ability to triage and prioritize patients in a high volume, fast-paced environment
  • Strong organizational, problem solving, and decision making skills
  • Demonstrated ability to work independently and collaboratively as part of an interprofessional team
  • Excellent interpersonal, verbal, and written communication skills
  • Ability to establish effective working relationships with patients, families, physicians, staff, and external partners
  • Demonstrated ability in teamwork, conflict resolution, priority setting, and time management
  • Commitment to high quality patient care, evidence-based practice, and professional standards
  • Ability to adapt positively to a fast-changing and stressful environment
  • Demonstrated satisfactory job performance and attendance
  • Commitment to promoting a healthy work environment
