Summary of responsibilities

Reporting to the Manager, Chronic Disease Management, the Nurse Practitioner (NP) provides direct, specialized, comprehensive nursing assessments, case management, lifestyle counselling, and education, emphasizing health promotion and disease prevention strategies to impact health outcomes positively. The focus of this position will be within the Respiratory Clinic assisting individuals with chronic lung health conditions. The NP coordinates patient care to facilitate continuity of care for patients living with Chronic Respiratory Conditions, working collaboratively with interdisciplinary teams to assess, plan, implement, and evaluate clinical interventions. The incumbent acts as a liaison and consultant to other healthcare providers and regional partners regarding patient care and health services within the health region.

Requirements of the job

  • Holds current Certificate of Competence from the College of Nurses of Ontario in the extended class
  • Baccalaureate Degree in Nursing from a recognized University
  • Ability to apply critical thinking to patient care
  • Demonstrated Knowledge and understanding of legal and ethical issues
  • Excellent organizational, interpersonal, problem solving and decision making
  • Demonstrated leadership ability, teaching and consulting skills
  • Excellent writing and publication skills
  • Clinical expertise related to patient care and administrative, educational and research matters
  • Strong clinical assessment skills, in particular in the area of gerontology and respiratory health
  • Advanced Knowledge of the consultation process as it relates to individuals, groups and/or

systems

  • Knowledge of the regional health care system in the area of the discipline/specialty
  • Knowledge of computers including MS Office (e.g. Word, PowerPoint, Access, Excel, statistical

programs, graphics packages)

  • Proven Ability to work effectively with all members of the healthcare team
  • Demonstrated regular, punctual attendance
  • Demonstrated commitment to patient safety
  • Functional level in both official languages (oral and written in English; oral in French and

testing completed)

Preferred:

  • Membership with RNAO and the Nurse Practitioner’s Association of Ontario
