We are searching for a compassionate, energetic, and community-minded Seniors Wellness Coordinator to join our Nursing Home Without Walls team based in Fredericton, New Brunswick.
Nursing Home Without Walls is an innovative program that takes support directly to older adults in their own homes. This role focuses on enhancing the social wellbeing, engagement, and overall quality of life of older adults through individualized wellness visits, meaningful group programming, and strong community connections. Key Responsibilities

Conduct one-on-one wellness and social visits with seniors living independently in the community
-
Build positive, supportive relationships with clients and promote social engagement and wellbeing

Plan, coordinate, and facilitate weekly group events, activities, and wellness programs for clients

Collaborate with recreation coordinators and community partners to develop meaningful programming opportunities
-
Support clients in accessing community resources and participating in social and recreational activities
-
Monitor client engagement and wellbeing, and communicate relevant updates to the Seniors Navigator and team

Assist with program setup, activity preparation, transportation coordination, and event logistics

Maintain accurate documentation and uphold client confidentiality and professional standards

Complete accurate documentation, evaluations, and participation records, and use data to support program development and continuous improvement.

Support a strong culture of safety by following all standard operating procedures, safe work practices, and contributing to a safe environment for clients, staff, volunteers, and students.
About You
In addition to placing high value on continuous improvement, collaboration and accountability, you bring:
- Post-secondary in a related field such as Gerontology, Psychology, Kinesiology, Recreational Therapy, Social Work, or a similar discipline
- Availability to work Monday – Friday
- Experience working with older adults in community, healthcare, recreation, or wellness settings
- Possession of a valid driver’s license and access to a reliable vehicle for travel to client’s homes
- Ability to write and speak English is required, French considered a strong asset;
- Ability to provide a clear criminal record and vulnerable sector check required upon hire;
- Current First Aid & CPR is required;
- Demonstrated passion for supporting the social well-being and quality of life of older adults
- Excellent interpersonal, communication, and relationship-building skills.
- Strong organizational skills and the ability to work independently and collaboratively within a team environment
- Commitment to representing the Nursing Home Without Walls program in a professional and compassionate manner
- Commitment to client-centered, respectful, and culturally sensitive care
- Demonstrated ability to adapt programs to changing client needs and behaviours
- Compassion, patience, and an empathetic approach to client care and support
- Creativity and enthusiasm in planning engaging activities and events
- Experience working in both one-on-one and group settings
- Knowledge of senior wellness, healthy aging, and community-based supports
