In accordance with the established vision, values, and strategic direction of the organization, the Community Health Worker II, as part of an interprofesional health care team, and under the direction of the Team Lead or designated clinical leader, provides home support services to clients in accordance with a pre-established care plan. Assists with activities of daily living such as bathing, grooming, oral hygiene, planning and preparing meals while promoting maximum independence. Performs delegated tasks for which delegation of tasks training has been completed. A key component of this job is to ensure the well-being of individuals and families and to promote independent living while in the community.

Must have current registration with the BC Care Aide Registry Shift Rotation/Hours of work Days, 08:00 to 16:45 (rotating)

About Hudson’s Hope Health Centre has an excellent health facility that serves a population of about 1,159 with an average of 800 ER visits per year. The clinic offers a range of services including:

  • Primary Care
  • Palliative Care
  • Home and Community Care
  • Laboratory
  • X-ray
  • Pharmacy
  • Dental Clinic

Hudson’s Hope is located on the banks of the Peace River in the Rocky Mountain foothills of British Columbia, 90 km west of Fort St. John and 66 km north of Chetwynd on Highway 29. A uniquely progressive and safe community, Hudson’s Hope is the third oldest European community in British Columbia. The area is one of the richest sites of fossils and dinosaur footprints in the world. The Hudson’s Hope Museum fossil display is the finest collection in the Peace River area.
