Summary of Duties
To provide a full range of social work services to patients and their families at the Holland Orthopaedic and Arthritic Centre. This includes psycho-social assessments, counselling to patients and families, discharge planning, crisis intervention, linkage to community resources, team consultation.
Qualifications/Skills
- Masters Degree in Social Work
- Member of the Ontario College of Social Workers and Social Service Workers
- Minimum two (2) years of experience working in a Hospital setting
- Exceptional communication skills required
- Familiarity with issues of capacity and consent
- In-depth understanding of the Sunnybrook Discharge Policy and its application
- Strong negotiation and conflict resolution skills
- Ability to be a team player as well as self-directed
- Ability to manage multiple tasks and set priorities in a fast-paced environment
- Commitment to student education
