About the role:

The Hospital strives to deliver care that is centred on patients and families. Patient and Family Centred Care (PFCC)

is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values

and ensures that those values guide all decisions. Creating a caring environment for patients and families ensures

continuity of care and fosters a learning environment. Applicants are required to have a demonstrated knowledge,

understanding and commitment to this care philosophy.

Under the general direction of the Implementation Coordinator, Medical Transitions Clinic (MTC), and accountable to

the Director of Acute Medicine, Stroke and Seniors' Program, the MTC clerk assists in the implementation and

administration of the program by performing clerical functions associated with the overall function of the clinic.

EMPLOYMENT REQUIREMENTS

* Post Secondary diploma/certificate in Secretarial Arts/Office Administration or equivalent experience (1 year

minimum).

* Minimum 2 years recent experience (during past 5 years) in a clinical care setting, preferably in an outpatient

program.

* Excellent telephone and interpersonal skills.

* Administrative and computer skills to include spreadsheets, word processing, and database applications (MS Word,

Excel, Publisher, Groupwise).

* Experience with Meditech Information Systems, specifically Community Wide Scheduling.

* Experience generating technical reports from multiple data sources.

* Data quality auditing experience.

* Extensive knowledge of medical terminology and standard abbreviations particularly in the complex medical field.

* Superior interpersonal communication skills (verbal, written, keyboard, telephone).

* Radiates a positive attitude and collaborative approach to interprofessional colleagues.

* Highly motivated & organized individual with the ability to prioritize activities independently.

* Excellent problem solving ability related to technical systems and potential process flow improvements.

Salary Range: $27.083 to 27.772
