The Opportunity As a Nurse Practitioner with REACH you will:
- Work in collaborative interdisciplinary team with a wide range of health professionals: physicians, nurses, social work, counselling, dental, pharmacy, health promotion professionals and more.
- Provide responsive, patient-centered, holistic urgent and primary care for patients from a diverse cultural and socioeconomic backgrounds with a wide range of health needs.
What You Bring
- Current registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
- 1 or more years of experience working as a Nurse Practitioner in a primary care setting.

REACH Urgent and Primary Care Centre is a department of REACH Community Health Centre, a dedicated health hub committed to making positive impact on comprehensive care. Located in the Grandview-Woodland area, REACH UPCC provides triage and delivery of urgent primary care services to any patient who presents themselves during the hours of operation, regardless of whether they are attached to another primary care provider for longitudinal care. A team of interdisciplinary supports includes 1 coordinator, 5 MOAs, 4 LPN, 4 RN, 2 NP, 4 MRP, 2 Social Workers, and several casual staff. While the emphasis is on urgent primary care, REACH UPCC strengthens our existing commitment to the social determinants of health as a foundation for patient care. We are also developing ways of connecting patients to care in the evolving Primary Care Network surrounding the REACH UPCC so that it functions as a hub and integrator in the network.
Hours
- Monday to Saturday, 8am-10pm
- Sunday and Statutory Holidays, 9am-5pm
