Role Description This full-time, on-site Occupational Therapist role is based in the Greater Toronto Area, Canada. The Occupational Therapist will assess clients in their homes and communities, identify functional challenges, and develop individualized treatment plans to enhance safety, independence, and quality of life. Daily responsibilities include conducting standardized assessments, recommending and training clients in the use of adaptive equipment, and providing therapeutic interventions such as activities of daily living (ADL) retraining, cognitive and perceptual rehabilitation, and environmental modification. The role involves collaborating with nurses, personal support workers, physicians, and other care providers to coordinate services, document clinical findings and progress, and educate clients, families, and caregivers on strategies to support safe, independent living. The Occupational Therapist will travel within the Greater Toronto Area as required to deliver in-person care.
Qualifications Current licensure or eligibility for licensure as an Occupational Therapist in Ontario; graduation from an accredited Occupational Therapy program. Clinical skills in functional assessment, ADL and instrumental ADL training, cognitive and perceptual evaluation, and therapeutic intervention planning. Experience with home safety assessments, equipment and assistive device recommendations, and environmental modifications to support independent living. Strong collaboration and communication skills for working effectively with clients, families, caregivers, and interprofessional health care teams. Proficiency in documentation, care planning, and use of electronic health records or clinical software systems. Ability to manage a community-based caseload, including scheduling, time management, and travel within the Greater Toronto Area. Demonstrated commitment to person-centered care, cultural sensitivity, and respect for diversity in home and community settings. CPR/First Aid certification and a valid driver’s license with access to a reliable vehicle are strong assets. Previous experience in home health care, rehabilitation, gerontology, or community-based practice is an asset.
