Description JOB DESCRIPTION The Ontario Shores seeks a Registered Practical Nurse for the Assertive Community Treatment Team, (HKPR). Reporting to the Clinical Manager, the successful candidate will have sound knowledge of clinical issues in the area of mental health and addiction.
In addition to doing mental health and physical assessments, responsibilities include: participating as a mental health professional on a multidisciplinary team, providing assertive community treatment to clients with severe and persistent mental illness; providing client-centred treatment rehabilitation and support to referred clients; developing treatment / rehabilitation goals and plans in collaboration with clients, their families, their support network and the multi-disciplinary team; providing clinical case management services (i.e. psychosocial interventions, environmental adaptations, delivering medication, monitoring medication compliance, assist and support clients in vivo with activities of daily living, harm reduction related to substance use, vocational rehabilitation, social relationship and use of leisure time); assessing for and recognizing signs of deterioration and institute actions to prevent further hospitalization; provide emergency crisis intervention services to help the client and/or others (i.e. family members, home operators, emergency room physicians) deal with emotional or behavioral crises involving other clinicians as appropriate.
Requirements
- Registration with the College of Nurses of Ontario in good standing
- Current CPR – Health Care Provider (HCP)
- Demonstrated knowledge of College of Nurses of Ontario (CNO) Standards of Practice and commitment to maintaining reflective practice standards as described by the CNO.
- Demonstrated knowledge of Canadian Standards for Psychiatric – Mental Health Nursing, and relevant policies and legislation, such as the Mental Health Act and PHIPA is required.
- Proven excellent interpersonal skills and effective oral/written communication skills.
- Implements and models the Recovery philosophy in all aspects of care as a Recovery Champion
- Demonstrated working knowledge / experience with computers
- Meditech Experience preferred
- You must have in-depth knowledge and experience of the challenges of acute psychiatry
- You must have skill and knowledge in the area of Psychosocial Rehabilitation and Recovery
- Mental Health experience >one year preferred
- Ability to work independently and within a multidisciplinary team
- Understanding of recovery principles and their application to people with mental illness both in hospital and community
- The ability to work with clients of diverse ethno racial and cultural backgrounds and proficiency in a second language would be considered an asset
- Proven good attendance record with the capability of maintaining this same standard
- Must be able to work all shifts and weekends
