Position: Friendly Connections and Wellness Program Coordinator

Position Type: Full-Time, Permanent

Hours of Work: 35 hours per week, In-Person Position.

Compensation: $21.12- $27.41 hourly

Position Summary

Responsible for the implementation, coordination and evaluation of Friendly Visiting, Security Check, and Wellness Program, health promotion outreach services; supervising volunteers and organizing training and orientation programs and activities related to community and program needs. The coordinator will also work in collaboration with internal partners to connect clients and caregivers to services as needed.

Responsibilities

Plans and conducts services related to the Friendly Visiting, and Health Promotion Departments Plans, coordinates and deliveries effective friendly visiting, telephone reassurance, friendly escorting, friendly shopping, security check services, and wellness activities. Assesses the needs of each client and develops a customized services plan with client Support clients to take an active role in their social, mental, and physical wellbeing Schedules visits and appointments within the scope of the services to optimize client safety Supervises and trains Friendly Visiting and Wellness Program volunteers to ensure effective service delivery Monitors the performance of each volunteer and maintains records of volunteers’ services Evaluates and improves Friendly Visiting and Wellness services outcomes Performs other support duties

Education

Diploma in Social Services or equivalent, preferably in with a focus in Gerontology Membership in good standing with a regulatory body in Ontario (Ontario College of Social Workers and Social Service Workers (OCSWSSW)

Skills & Experience

Fluency in spoken and written Cantonese and or Mandarin is required.

One year’s experience in social services

Experience in program planning, implementation and evaluation and group facilitation Experience in working with volunteers Proficient in Microsoft Office, specifically Word and Excel Experience working in diverse communities, with the ability to be culturally sensitive and appropriate is an asset Ability to identify needs of seniors, particularly high risk, frail, vulnerable clients Counseling, referral and advocacy skills Excellent communication skills delivered in an empathetic and caring manner Ability to work both independently and as part of a team

Working Conditions

Required to conduct home visits with clients and caregivers; working alone in the home Regular interruptions to support clients’ needs and crises Regular exposure to computer terminal

A valid driver’s license, and access to a vehicle are required for home visits.
