About the role
QUALIFICATIONS
- Eligible Baccalaureate Degree in Nursing
- Active practicing licensure as a Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) in Manitoba
- Minimum five (5) years of nursing experience, including three (3) years in Community Health, Primary Care, Emergency, or Substance Use Services
- Other combinations of education may be experience
- Current Basic Cardiac Life Support (BCLS) Training as delineated in Prairie Mountain Health Policy PPG-00002 CPR Training
- Province of Manitoba Class 5 Drivers Licence, or equivalent from province of residence, and access to a personal vehicle to provide service within Prairie Mountain Health
- Current certification:
- ASIST (Applied Suicide Intervention Skills Training)
- NVCI (Nonviolent Crisis Intervention)
- CODEC (Co-occurring Disorder Education Curriculum Tier I and II)
- Proficiency with Microsoft programs (Outlook, Word, Excel, Access and PowerPoint), as well as Internet applications and other Information Technology.
- Evidence of professional development and activity in a professional association
- Knowledge of adult learning principles and ability to teach individuals and groups effectively
- Knowledge of, competence and aptitude for working with this specific patient population
- Demonstrated skills in assessment, problem-solving, communication, and cultural safety
- Demonstrates knowledge of trauma-informed care and recovery-oriented philosophy of mental health

POSITION SUMMARY:
The Nurse III provides integrated substance use and mobile withdrawal management services across Prairie Mountain Health, supporting individuals with substance use challenges through direct care, education, and system navigation. This role is central to enhancing clinical pathways between RAAM, Primary Care, MOST and community supports, and delivering mobile non-medical withdrawal management services (MWMS).
The nurse works collaboratively with Primary Care Providers (PCPs), RAAM teams, MOST, and community agencies to facilitate transitions of care, support opioid agonist therapy (OAT), and deliver culturally safe, recovery-oriented services.
RESPONSIBILITIES:
Client/Community Services

Deliver Substance Use Care and MWMS Provide evidence-informed care to individuals and groups experiencing substance use challenges, including support for Opioid Agonist Therapy (OAT). Monitor client progress, manage medication adherence, and respond to emerging health needs in collaboration with interdisciplinary teams.

Conduct Comprehensive Substance Use and Mental Health Assessments and Develop Culturally Safe Care Plans Perform biopsychosocial assessments that incorporate trauma-informed and culturally appropriate approaches. Collaborate with clients, families, and community supports to co-create individualized co-occurring care plans that reflect client goals, strengths, and social determinants of health.

Provide Mobile Services in Diverse Settings Deliver virtual and in-person services across a range of environments, including homes, shelters, community agencies, and other non-traditional settings. Adapt care delivery to meet clients where they are, ensuring accessibility and continuity of care.

Facilitate Transitions from RAAM to Primary Care Support seamless transitions for clients moving from Rapid Access to Addictions Medicine (RAAM) services to ongoing care in primary care settings. Ensure transfer of relevant clinical information, promote continuity, and reduce barriers to engagement.

Act as a Liaison Between RAAM, Primary Care, and Community Supports Build and maintain collaborative relationships with primary care providers, mental health teams, housing services, and other community agencies. Serve as a connector to ensure integrated care pathways and timely access to resources.

Ensure Safe MWMS and Timely Referrals Provide Mobile Withdrawal Management Services in safe and supportive environments, monitoring for complications and ensuring rapid referral to inpatient or residential treatment programs when clinically indicated.

Deliver Education and Distribute Clinical Tools to Primary Care Providers Conduct outreach and education sessions for primary care teams on substance use care, MWMS protocols, and OAT management. Provide standardized clinical tools and resources to enhance provider confidence and capacity.

Promote OAT Prescriber Training and Support PCPs Encourage and facilitate training opportunities for primary care providers to become OAT prescribers. Offer ongoing mentorship and consultation to support safe and effective management of clients receiving OAT in community settings.

Monitor and Report Key Performance Indicators Track client outcomes, service utilization, and system-level metrics such as transitions to primary care, provider engagement, and program effectiveness. Use data to inform quality improvement initiatives and support accountability to funders and stakeholders.
