Position Summary The Health Information Management Professional is responsible for the release of personal health information in accordance with the Personal Health Information Protection Act, 2004, and hospital policies. In addition, the position of Health Information Management Professional is to abstract and code patient information based on ICD-10/CA/CCI. The Health Information Management Professional is responsible for the interpretation of source documents located in the patient medical record and applies code assignment by selecting and assigning numeric codes from the classification systems to represent diagnostic and intervention data. Accurate code assignment involves interpretation and application of coding instructions, guidelines, conventions, standards, policies, rules and ethics.
Position Requirements
- Prepare and process personal health information
- Process correspondence and release personal health information
- Respond to the information requirements of Coroner's investigations, situations of child abuse, transfers, and other urgent situations
- Be familiar with statutory and legislative requirements of personal health information
- Code and abstract patient information in accordance with CIHI guidelines, hospital policies and procedures, and established hospital guidelines
- Input, process, update, correct, verify, monitor requests for follow-up
- Read and interpret health record documentation to identify all diagnoses and procedures
- Committee and Care Team involvement
Mandatory Qualifications
- Must have a CHIM certification
- Highly motivated and customer oriented
- Ability to co-ordinate multiple initiatives effectively, including the initiation and positive paradigm of change
- Knowledge of computerized : Admissions, MRI, Abstracting Grouper and Report Writer Systems
- Strong organizational skills
- Excellent oral, interpersonal, and written communication skills
- Intermediate computer skills, utilizing Microsoft software such as: Word, Excel, PowerPoint, Outlook
- Canadian College of Health Information Management
- Current registration with the Canadian Health Information Management Association (mandatory)
- Ontario Health Information Management Association (optional)
