What We Do:
- Holistic Rehabilitation: We provide complex assessment and treatment of acquired brain injuries, spinal cord injuries, amputations, chronic pain, orthopaedic injuries, psychological impairments, and learning/developmental disabilities! We are hiring for two separate sectors: 1) Auto-sector, WSIB, LTD, and Veteran's affairs. 2) Public sector Autism/Developmental Disability interventions; Private pay pediatrics in clinic (St. Catharines).

Job Overview: We are seeking a highly skilled and compassionate Certified Life Care Planner (CLCP) to join our team. The ideal candidate will possess a thorough understanding of medical conditions, treatment plans, and long-term care needs for individuals with chronic illnesses, disabilities, or catastrophic injuries. As a CLCP, you will collaborate with healthcare professionals, attorneys, and insurance companies to develop comprehensive life care plans that address the current and future needs of patients, ensuring they receive the highest quality of care possible.
Key Responsibilities:
- Conduct detailed assessments of individuals with catastrophic injuries, disabilities, or chronic conditions to identify their current and future healthcare needs.
- Develop individualized life care plans, including medical treatments, therapies, assistive devices, home modifications, and ongoing care requirements.
- Coordinate with multidisciplinary teams, including physicians, rehabilitation specialists, case managers, and attorneys, to ensure an integrated approach to care.
- Research and compile accurate, evidence-based recommendations for long-term care strategies, including cost projections for future medical care.
- Collaborate with families and caregivers to educate them on the proposed life care plan and support their understanding of long-term care requirements.
- Provide expert testimony in legal proceedings, including depositions, trials, and arbitration, regarding life care planning assessments and recommendations.
- Maintain comprehensive, detailed, and up-to-date records of life care plans and case progress.
- Stay current with industry standards, medical advancements, and regulatory changes related to life care planning.
- Ensure compliance with ethical standards, legal regulations, and professional guidelines related to life care planning.
Qualifications:
- Certification: Must be a Certified Life Care Planner (CLCP) through the Commission on Rehabilitation Counselor Certification (CRCC) or a recognized certifying body.
- Education: Bachelor's or Master's degree in nursing, healthcare, rehabilitation, or a related field (required).
- Experience: Experience in life care planning, case management, or a related field
- Knowledge: Extensive knowledge of medical terminology, disabilities, and rehabilitation therapies.
Skills:
- Strong analytical skills to evaluate medical records and assess patient needs.
- Excellent written and verbal communication skills for report writing and presenting complex information to diverse audiences.
- Ability to work effectively within a multidisciplinary team.
- High attention to detail and the ability to manage multiple cases simultaneously.
Certifications: CPR and First Aid certifications preferred.
Preferred Qualifications:
- Experience working with catastrophic injury or trauma cases (Motor Vehicle Accidents).
- Familiarity with legal aspects of life care planning, particularly in relation to personal injury law, worker's compensation, and insurance claims.
- Experience in conducting life care planning assessments for both pediatric and adult populations.
Additional Information:
- This position may require travel to client homes, healthcare facilities, or courtrooms.
- Work schedule may vary based on client needs and deadlines.

Our areas of clinical practice include acquired brain injury/ neurological impairments, severe orthopedic injury (spinal cord injury/amputation/fractures), psychological disabilities and chronic pain. We provide hospital discharge planning services, followed by community based individualized care planning. There are a multitude of different types of intervention services our team provides including: home/vehicle accessibility, equipment prescription, community safety/reintegration, wheelchair positioning/ seating prescriptions, activities of daily living (ADL) re-training/ functional restoration programming, cognitive remediation/compensatory therapies, cognitive behavioural therapy, quality of life and return to modified sport programs. Our team is additionally skilled in vocational services, including return to work programs, work-site, vocational exploration and ergonomic assessments.
