POSITION OBJECTIVE
The Southern Network of Specialized Care requires one (1) full-time contract half (.5) Dual Diagnosis Justice Case Manager/ half(.5) Complex Support Coordinator to work in the Brant, Hamilton, Haldimand, Norfolk, and Niagara communities.
Working to promote Bethesda’s values, vision and mission statement and quality client care, The Dual Diagnosis Justice Case Manager will provide case management working with justice, mental health and development services to develop case plans for high needs individuals involved with justice, mental health and developmental service sectors.
The Complex Support Coordinator will provide intense evidence based person centered coordination of supports including development of plans and linkages to services for persons with complex multisystem needs who require a high level of active engagement and ongoing monitoring. The Complex Support Coordinator position requires a high degree of problem solving, communication and clinical acumen. Effective collaboration is critical and the Complex Support Coordinator is responsible for ensuring a high level of quality in service, including strong collaborative relationships with service providers and stake holders. The Complex Support Coordinator will ensure adherence to the CNSC mandate.
Key Job Responsibilities
- Coordinate the discharge planning process by partnering and planning with mental health court support workers, local correctional workers and custody facilities.
- Coordinate a continuum of services based on client choice and need; act as the central contact person to be referred to mental health services and other supports; work with individuals receiving service to maintain community involvement with adequate supports.
- Act as a link between the Southern Network of Specialized Care in the region and the developmental services case management supports at the community level to facilitate longer term support for individuals.
- Participate in regular meetings with mental health court workers to share information and identify issues and strategies to improve their services.
- Act as a resource to justice and mental health systems regarding service delivery and gaps in the developmental service and provide input into resolving process issues.
- Plan for ongoing Ministry of Community and Social Services case management services with local communities for individuals.
- Identifies, informs and facilitates the development of a plan that will address the mental health needs, developmental and community support needs at key intervention points in the judicial process.
- To provide transitional support to the target population.
- Responsible for reporting all health and safety concerns immediately following the Internal Responsibility System.
- Complex support coordination in accordance with the CNSC model.
- Provide person centered planning, including transition planning, by leading, developing, facilitating, coordinating, implementing, monitoring (both goal/ outcome achievement and service gaps) and updating person-centered plans through collaboration with the individual, caregiver and other stakeholders.
- Promote person centered practices including use of person centered tools in planning.
- Facilitate access to specialized resources, including the local, regional and provincial levels.
- Utilize established linkages within the SNSC & CNSC to assist with exploring local, regional and provincial resources for persons requiring complex case coordination.
- Provide cross sector coordination that supports case resolution including service navigation, liaising with cross sectoral service partners, coordinating referrals and support and when necessary advocating for required supports and services.
- Balance the need to attend to both individual/community safety considerations and promoting self-advocacy and self-determination.
- Support clients and families that are adapting to change or are in crisis and are having difficulty coping, making decisions and problem solving.
- Develop appropriate strategies for clients who may be resistant to services and who live at risk in the community.
- Collaborate with the Hamilton Niagara Brant Haldimand Norfolk Developmental Services Ontario coordinate information sharing required to support the CNSC model.
- Collaborate with other case management supports and community based agencies to ensure efficient use of resources in complex support coordination.
- Represent the SNSC in a positive and professional manner.
- Complete in a timely and professional manner all documentation requirements.
- Maintain a current knowledge of cross sectorial community resource.
- Participate and contribute in regular supervision meetings, team meetings and other program meetings.
- Comply with Bethesda policies and procedures relating to health and safety.
- All other duties as assigned.
QUALIFICATIONS
- Degree in Social Services, Social Work, Criminology, Justice or other related field.
- A minimum of five years previous experience working within the Developmental Services Sector, Justice and/or Mental Health field with advanced knowledge and skills related to developmental disabilities, mental health, family work, and systems issues;
- Professional Communication Skills: professionally represents the company through strong communication skills, both oral and written.
- Teamwork: excels at working with others and communicating with a team to provide the most effective support to clients.
- Problem-Solving: able to navigate challenges and barriers, always looks for potential pitfalls and risks to help mitigate problems ahead of time, and knows when to ask for help.
- Proactive: anticipates and solves problems and issues before they are brought to your attention
- The preferred candidate maintains professional membership in good standing with a regulated college.
- Knowledge of application of case management principles acquired and maintained through membership in good standing with a regulated college
- Demonstrated complex support coordination abilities and a familiarity with how to access support and services available to adults with a developmental disability and complex medical and/ or behavioral support needs;
- Demonstrated ability to work in a cross sector context that builds upon a systemic framework and promotes cross sector collaboration and partnerships;
- Organizational skills and demonstrated ability to handle and respect confidential information with tact and diplomacy;
- Demonstrated ability to adhere to deadlines and prioritize multiple projects effectively;
- Demonstrated ability to independently plan and prepare presentations and proposals;
- Demonstrated ability in adherence to client rights and an understanding of client’s ability to direct care and client confidentiality;
- Knowledge of a variety of computer software applications and automated record keeping;
- A valid province of Ontario’s driver’s license and access to a reliable and insured automobile.
