Compensation: $89,508.74 – $105,520.37plus a comprehensive compensation package that includes time-off entitlements, health benefits, life insurance, and participation in the HOOPP pension plan.

We are Somerset West Community Health Centre (SWCHC), a non-profit, community-governed organization that serves over 18,000 clients annually. Established in 1978, SWCHC provides a wide range of health care services such as primary health care, health promotion, community development, mental health, and harm reduction. What the Program Entails

The Family & Community Services department supports community wellbeing through integrated health and social services, health promotion, community development, and equity-focused initiatives. Within this department, the Black Health portfolio advances culturally responsive, community-informed approaches that improve access, coordination, and health outcomes for Black communities in Ottawa. This work aligns with SWCHC’s mission, the organization’s strategic priorities, and broader provincial efforts to advance Black Health equity.

What You’ll Do

Reporting to the Director, Family & Community Services, the Manager – Equity and Integration, Black Health helps advance SWCHC’s equity commitments in the area of Black Health. This role works across the organization to integrate and coordinate health and social service supports for Black communities. The role also aligns with the Black Health Committee’s (BHC) provincial Black health strategy and identifies opportunities to advance Ontario Health’s Black Health Plan.

What We’re Looking For

Life and Work Experience

  • Experience working within health service delivery focused on priority populations including Black health & wellness initiatives

  • Demonstrated experience and understanding of community development, social determinants of health, anti-oppression and health equity

  • Relevant and significant coordination, program or project management experience.

Your Education Path

  • Master’s Degree in relevant discipline (social work, sociology, public health, etc.) or a combination of undergraduate degree and significant community development experience.

Required Language

  • Fluency in English is required.

  • Working knowledge of French or other languages are an asset.

  • Excellent time management skills and project management methodologies.

  • Ability to work under pressure and deal with conflicting priorities and deadlines with the highest degree of accuracy.
  • Ability to monitor own performance objectively.
  • Ability to evaluate own needs for personal and professional development to maintain a high level of competence and practice.
  • Demonstrated complex decision-making ability and creative problem solving / critical thinking.
  • Demonstrated ability to persuade and positively influence others.
  • Strong leadership skills are required: people leadership, coaching, decision making, and motivation
  • Demonstrated experience in the development and implementation of population health focused initiatives, programs and outcomes
  • Excellent interpersonal and collaboration skills with a proven ability to successfully deal with complex issues working within the context of cross-functional teams
  • Strong verbal, written, presentation (PowerPoint) and interpersonal skills

Your Day-to-Day at a Glance

As our Equity and Integration, Black Health Manager, your responsibilities are:

  • Lead the continued implementation of SWCHC’s Black Health Plan activities and objectives in alignment with the provincial mandate and the organization’s strategic priorities.
  • Work with internal programs to strengthen referral pathways, coordination, and community engagement efforts that improve access to culturally responsive health and social services for Black communities.
  • Build and maintain relationships with external partners to strengthen supports related to chronic disease management, mental health, and Black women’s health.
  • Provide leadership to increase organizational awareness of Black Health priorities and support the advancement of equity-focused initiatives across teams and services.
  • Identify and support education, health promotion, and system navigation opportunities that respond to the needs of Black communities, including equity-deserving groups within these communities.
  • Participate in and support internal and external partnerships, stakeholder meetings, and broader system collaboration initiatives.
  • Collaborate on internal and external communications to promote awareness of Black Health priorities, initiatives, and community engagement activities.
  • Support data-informed planning, program monitoring, evaluation, and performance reporting related to Black Health initiatives.
  • Contribute to organizational planning, continuous improvement, and team effectiveness through leadership, coordination, and collaboration.

Somerset West Community Health Centre (SWCHC) is a interprofessional agency offering a diverse spectrum of programming, in order to promote optimal health and social well-being among community members. Priority is given to individuals with unique needs, including single parent families, those who are financially insecure, new Canadians, and persons coping with various disabilities. SWCHC welcomes and supports individuals of all gender identities, sexual orientations, ages, and ethnicities.

SWCHC serves a large multicultural area in Ottawa's centre west, including Dalhousie, Hintonburg, Mechanicsville, and West Wellington, with a population of approximately 40,000 persons. Much of the community's population is made up of new immigrants to Canada, who face many struggles which for many include a lack of English language skills.
