In this blended role, you will provide OTA/PTA & Personal Care Support as part of an interdisciplinary team that uses an integrated and collaborative approach to service delivery in the Wellington County Region!. CBI Home Health has demonstrated that In-Home care can be diverse and individualized with evidence of improved outcomes and satisfaction for personnel, patients and families. CBI Home Health has strengthened the traditional In-Home service delivery model by focusing on a holistic approach to care. As part of the dynamic and self-directed care team, you work together to effectively meet the patient’s goals. This role requires a driver's license and reliable vehicle to travel to clients homes within the East Wellington Region including rural areas such as Rockwood, Mapleton, Minto, Drayton and Mount Forest.

What you need to be successful
- You possess a post-secondary diploma as an Occupational Therapist Assistant/ Physiotherapy Assistant (OTA/PTA)
- Valid Driver's License, Vehicle Insurance, and reliable vehicle is required
- Travel required within rural communities in East Wellington (Rockwood, Mapleton, Minto, Drayton & Mount Forest)
- You have a general knowledge of mobility aids and assistive equipment
- Your focus is to embrace and support person and family-centered care
- You are passionate, empathetic, use good judgement and apply critical thinking when making decisions
- You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
- You value the role a therapist plays in keeping clients at home when that is of personal importance
