The Role

As a CFF Care Coordinator, you will play a key role in supporting children, youth, and families experiencing mental health challenges by developing and coordinating individualized, high-fidelity wraparound care plans. Working as part of a family-centered, multidisciplinary team, you will serve as the main point of contact for care teams, facilitate collaboration with community partners, and ensure services are well-integrated and responsive to each family’s unique needs. This role requires strong relationship-building, problem-solving, and coordination skills, with a deep commitment to family engagement, safety planning, and inclusive service delivery.

Please note that this is a Permanent full- time position, working 35 hours weekly,

Who we’re looking for:

  • A degree in Social Services or a related discipline from a recognized university, or a college diploma in a related field.
  • Minimum of three (3) years of experience working in children’s mental health, social services, or primary health care settings.
  • Experience in facilitating meetings with diverse stakeholders.
  • Established working relationships with Halton community service providers is an asset.
  • A highly collaborative leadership style with the ability to work respectfully and effectively across systems that support children, youth, and families.
  • Strong verbal and written communication skills.
  • Excellent problem-solving and conflict resolution abilities.
  • Exceptional organizational and time management skills.
  • Proficiency in using technology, including working in virtual environments; experience with MAC OS operating systems is preferred
