Waypoint Centre for Mental Health Care is a 315-bed academic and teaching hospital that provides specialized mental health, addiction and geriatric care. Located on the shores of Georgian Bay, we serve some of the most complex and disadvantaged individuals in Ontario and are the sole provider of high-secure forensic mental health services in the province.
The Manager of Professional Practice and Quality is currently seeking a temporary full-time Patient Safety Quality Improvement Coordinator. The purpose of the position is to coordinate and integrate hospital-wide patient safety (PS) and continuous quality improvement (CQI) initiatives which include but may not be limited to managing and analyzing quality/performance indicators in compliance with Accreditation Canada (AC) standards, Excellent Care for All Act (ECFAA), and applicable regulations.

KEY ACCOUNTABILITIES:
- Coordinate hospital-wide PS and CQI initiatives incorporating the use of performance/outcome indicators; accreditation advisory and quality committees; updating and maintaining patient safety and quality improvement policies and procedures; providing presentations and quarterly reports to director, senior managers and governing body regarding PS/QI and/or service evaluation activities; collaborating with managers and the Decision Support Team regarding the collection and reporting of indicator data on a hospital-wide basis, and; providing advice, support, coordination, and education re the hospital’s accreditation preparation process.
- Conduct, analyze and report patient/client incident data and/or quality performance indicators registered in the electronic database and making recommendations re ways to minimize/mitigate patient safety concerns through system changes and improvements and/or discussing and offering solutions when possible, to develop, review and revise facility policies as appropriate to maintain adherence to current standards and regulatory requirements.
- Maintain and upgrade of incident/safety data collection software.
- Provide consultative support for patient-care teams, support teams and departments to develop and explore patient/client safety strategies and/or CQI project ideas; facilitating project meetings and team formation; linking facilitators and sponsors with project teams; following-up on projects/activities; encouraging communication of quality improvement efforts; providing advice on PS and CQI practices/principles to departments, teams and managers.
- Assist in the maintenance of patient complaint/safety incident data files in compliance with regulatory agencies; promoting maximum confidentiality in keeping with relevant privacy protection legislation by limiting access to such information; collecting, evaluating and distributing relevant data concerning patient injuries, aggregate data summaries, monthly trend analyses of incidents; and monitoring resolutions to reported incidents.
- Participate in the review of adverse events and quality of care concerns – facilitating root cause analyses, conducting investigations, monitoring action plans and improvement efforts. Participating in ethical deliberations as appropriate. Directly refers to manager and/or relevant senior manager(s), issues of concerns related to quality of care
- Assess the educational/training needs of staff and teams related to patient safety and continuous improvement principles, methods and tools, and service evaluation activities; developing and updating education/training plans as required; delivering or coordinating the delivery of relevant learning activities to support PS/QI and evaluation activities; coordinating the hospital’s annual Patient Safety Week.
- Sustain relationships with PS/QI network of professionals both internally and externally in the broader healthcare sector through excellent verbal and written communication skills, and maintains a comprehensive knowledge re PS/QI and service evaluation literature, concepts, practices and principles.
REQUIREMENTS/QUALIFICATIONS:
- Degree in a healthcare discipline is required.
- Certification and/or diploma in Quality Assurance and Patient Safety is an asset.
- Results oriented work ethic to work independently and undertake tasks needed to accomplish work objectives and deliver quality, consistent and timely results
- Demonstrated leadership, conflict resolution, interpersonal and organizational skills
- Proven written and verbal communication skills
- Proven ability to adapt readily to change and reflects an optimistic and positive attitude
- Models and promotes core ethical practice, Waypoint Values (Caring, Respect, Accountability, Innovation)
- Preference for proficiency in French/English language skills.
