About the role

Your Opportunity:

The Registered Nurse is responsible for the assessment, planning, implementation and evaluation of comprehensive patient and family care in the Heart Function Stabilization Clinic (HFSC) at the CK Hui Heart Centre. This role requires independence and self-direction and adheres to a case management structure in accordance with RAH and AHS policy and procedures. Patients of the HFSC present with multiple signs and symptoms related to heart failure and require a multidisciplinary approach to meet their health needs. This collaborative team is made up of Registered Nurses, dieticians, administrative support staff, Physicians, and a Nurse Practitioner. Duties include, but are not limited to; completion of an initial comprehensive cardiac assessment, monitoring/evaluating and addressing the effectiveness of medical interventions/lifestyle changes, monitoring/addressing concerns with patient lab work, and presenting clinical findings to physicians and/or the nurse practitioner. Physical demands of the job include sitting for extended periods of time and using repetitive motions to complete typing/computer/telephone work daily. Although registered nurses in this setting work in close collaboration with the Cardiologist and Nurse Practitioner, independence and self-direction are key qualities of the successful applicant. Experience working in an Outpatient Department, navigating the Connect Care framework, and a focus on adult health education is a requirement for this role. Strong knowledge base of the cardiovascular system and cardiac patient management is essential. Successful candidates will be expected to participate in quality improvement initiatives at the front line; supporting and maintaining high quality patient care strategies within the cardiology program.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.

  • CRNA registration required and maintained throughout duration of employment.
  • CPR - HCP level required.
  • A minimum of 5 years recent (within last 2 years) cardiology experience required within the following departments; CICU, CVICU, Cardiac Surgery, Cardiology Inpatient Ward, Cardiac Outpatient Department (Heart Function Clinic, Cardiac Specialty Clinic).
  • Preference will be given to applicants with Outpatient Cardiac Clinic experience; preferably within a Heart Function Stabilization Clinic setting.
  • Nurses are required to have a minimum of 2 years recent experience assessing, monitoring, and teaching adult patients with heart failure.
  • Demonstrated excellent organizational, interpersonal, communication and priority setting skills required.
  • Demonstrated reliability required.
  • Basic computer skills are required.

Preferred Qualifications:

  • Able to work collaboratively in a multidisciplinary team environment.
  • Understanding of adult teaching principles preferred.
  • Connect Care familiarity as asset.
  • Candidates may be required to write a medication calculation exam on interview. Failure to achieve a passing mark on the exam may impact decision to hire.
