Job Overview:
Reporting to the Community Health Clinic Manager, the Referral and Medical Documents Clerk plays a vital role in supporting the delivery of coordinated, timely care across The Alex’s multidisciplinary teams. The Referral and Medical Documents Clerk ensures smooth processing of medical referrals, secure handling of health records, and accurate information management within the Electronic Medical Record (EMR) system. The clerk supports various programs including Community, Youth, Seniors, Housing, and Mobile Health, facilitating internal and external referrals, and assisting with privacy-related documentation processes.
This is a full-time, permanent position.
Key Responsibilities:
Referral Coordination
- Manage all referrals within the EMR system, including entry, tracking, coordination, and reconciliation.
- Collaborate with physicians and nurse practitioners to select appropriate referral locations, considering accessibility, specialty, and client needs.
- Process referral documentation in accordance with program-specific criteria.
- Coordinate appointment bookings with internal specialists and follow up with clients to confirm or reschedule as needed.
Medical Documents Management
- Assist in processing of incoming medical documents, including laboratory results and consultation reports, ensuring timely EMR entry and linking.
- Collaborate with The Alex’s Privacy Officer to assess, redirect, or securely dispose of documents received in error, protecting client and organizational confidentiality.
- Responsible for responding to medical information requests in accordance with HIA and PIPA legislation, compiling and redacting records as necessary.
- Coordinate billing for release of medical records in collaboration with finance.
- Maintain detailed documentation of actions taken regarding health information.
Clinical Operations Support
- Respond to evaluation and data requests from clinical and administrative teams.
- Collaborate with the clinical and allied health teams to ensure efficient, client-centered service delivery.
- Cross-train to become an advanced EMR user and support other central operations functions as required.
- Perform other duties as assigned.
Qualifications and Experience:
- Medical Office Assistant or Unit Clerk Certificate.
- Minimum two years of experience in a medical office or healthcare setting preferred.
- Knowledge of medical terminology and experience handling confidential health documents.
- Strong understanding of CHR systems and Microsoft Office Suite.
- Understanding of complex care, mental health, addictions, and trauma-informed practice is preferred.
- Familiarity with social services and non-profit healthcare is an asset.
- Excellent organizational skills with the ability to multi-task in a fast-paced environment.
- Ability to work independently and as part of a multidisciplinary team.
- First Aid (Basic or Intermediate) – Level C CPR & AED within 30 days of hire date; certification must be valid for a minimum of 6 months upon hire.
- A satisfactory vulnerable sector criminal record clearance and a child welfare intervention check are required.
- Demonstrated knowledge, understanding and commitment to the Truth and Reconciliation Commission of Canada: Calls to Action is preferred.
Work Environment and Hours:
- The Alex Community Health Centre: 2840 2nd Ave SE, Calgary, Alberta
- Standard Hours: 8:30 am to 4:30 pm, Monday to Friday.
- Permanent full-time position
- 1.0 FTE (37.5 hrs/wk)
- Continuous extended periods of sitting, with some standing and walking (sometimes up or down stairwells).
