POSITION: Nurse Practitioner (NP)

LOCATION: Rekai Centre at Sherbourne Place
Seeking a Nurse Practitioner for in-person, on-site position at The Rekai Centre at Sherbourne Place. This is a Class C, 90 bed Long Term Care Home located Downtown Toronto. No Hybrid, No Remote position
Education and Specific Job Requirements
Required
- Legally entitled to work in Canada.
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class Primary Health Care or Adult and entitle to practice with no restrictions/limitations
- CPR Designation designed for HealthCare Professionals and Standard First Aid
- Advanced knowledge and decision-making skills in assessment, diagnosis, and health care management and treatment of older persons with delirium, depression, cognitive impairment, confusion and behavioural disturbance
- Current membership in professional associations: Nurse Practitioners' Association of Ontario (NPAO)
Considered an Asset
- Certification in Geriatric Nursing from the Canadian Nursing Association or equivalent experience
- Specialty Certificate – Nurse Practitioner Primary Health Care (Canadian certified)
- Certifications obtained or in progress that include gerontology, mental health, behavioral support and palliative care.
- Working Knowledge of the; Fixing Long Term Care Act, Program Standards for Long Term Care, Health Disciplines Act, Occupational Health & Safety Act, Pharmacy Act, Food and Drugs Act (Canada), Narcotic Control Act (Canada) and Coroners Act
Relevant Experience
- Two-Three (2-3) years’ clinical experience as a Nurse Practitioner in Canada
- Teaching experience will be considered an asset
Working Environmen t: working in close proximity to individuals exhibiting responsive behaviours related to diagnosed neurological conditions, compounded by mental health disorders and substance use issues
Responsibilities:
- Conduct comprehensive health assessments of residents, including physical examinations, medical history reviews, and medication reconciliation.
- Develop and implement individualized care plans based on residents' needs, goals, and preferences.
- Provide direct clinical care, including ordering and interpreting diagnostic tests, prescribing medications, and performing minor procedures within the scope of practice.
- Collaborate with physicians, registered nurses, allied health professionals, life enrichment professionals and other team members to optimize resident outcomes and quality of life.
- Educate residents, families, and caregivers on health promotion, disease prevention, and self-management strategies.
- Participate in interdisciplinary meetings, case conferences, and quality improvement initiatives to enhance resident care and safety.
- Maintain accurate and up-to-date documentation of assessments, interventions, and outcomes in electronic health records.
- Adhere to professional standards, regulatory requirements, and best practices in nursing and healthcare delivery.
- Support the Emotion-based care approach and the 2SLGBTQ+ initiative.
- Mentor Students
- Other Activities – Other activities may be assigned in collaboration with the inter- professional team
Skills, Knowledge Requirements

Computer Skills
- Electronic documentation of health records using a Clinical Management System CMS
- Knowledge of and working experience with computer systems and applications
Language Skills: English required. French not required but would be an asset.
Reporting Structure
The NP reports to the Executive Director for day-to-day operations. Collaborate with the Director of Care (DOC) and Medical Director (MD) to help support and align with our resident care needs.
