Role Summary
As the Care Planner, you will be the primary architect of client care journeys—from initial assessment through to implementation, ongoing review and optimization. You’ll coordinate resources, design care teams, align with clinical & operational standards, and serve as the key point of contact for clients, families and caregivers. You’ll also partner with business development and community-relations efforts, helping to grow our service footprint in the region. This is a full time,40 hrs/wk position.
Key Responsibilities
Client Intake & Assessment

Lead in-home consultations with referred clients and families to determine needs, preferences, goals, risks and home environment.

Use evidence-based tools and clinical judgment to identify relevant supports and create a full intake profile.

Develop and present detailed care plan based on client needs.

Attend caregivers’ initial visit with new clients.

Ensure all documentation is complete and securely stored.
Care Team
-
Select, onboard and match caregivers to the client.

Notify clients and caregivers regarding initial and ongoing schedule changes
-
Coordinate with scheduler/operations to ensure coverage and smooth transitions.

Responsible for overseeing, supervising caregiver’s delivery of care and training.
Implementation & Monitoring
-
Conduct periodic home visits to observe service delivery and ensure satisfaction.

Monitor outcomes and trigger plan revisions as required.

Conduct safety audits and update care plans accordingly
-
Document all interventions and communications in CRM software Alayacare.

Maintain knowledge of seniors’ care regulations and standards in BC.
Business & Community Relations
-
Build relationships with hospital discharge planners, assisted-living facilities, and healthcare partners.

Participate in community events and educational sessions.

Collaborate with marketing efforts to convert leads and increase referral.
Continuous Improvement & Analytics
-
Monitor metrics such as satisfaction, retention, and conversion.
-
Recommend process improvements for intake, plan design, and training.

Stay current on trends, technology, and market developments.
Required Qualifications
-
Diploma or degree in Nursing (LPN/RPN), Social Work, OT/Physio, or equivalent.

Minimum 2 years’ experience in home care, case management, or community health.

Success in sales in medical sector would be an asset.

First-aid/CPR and safety certifications.

Strong assessment, planning and documentation skills.

Valid BC driver’s license and vehicle for local travel.

Proficiency in Microsoft Office, CRM platforms.
