About the role
Overview
Community Outreach Services provides evidence based and high quality mental health support to individuals and families living in in East Toronto. As an essential member of the multi-disciplinary team, the Community Support Counsellor will join the REACHintensive case management service with a primary focus of providing high quality and evidence informed case management, supportive counselling and skill teaching to clients with severe and persistent mental illness in the environment of the client's choice. The Community Support Counsellor will carry a client caseload and work collaboratively with other team members to support client care. The Community Support Counsellor may also offer client groups as appropriate.
Education
Baccalaureate in a related field plus a college diploma in Human Services with a major in mental health. Evidence of ongoing commitment to professional development
Qualifications
Must have a minimum of three (3) years front line experience working in the area of community mental health support Demonstrated knowledge of the Recovery Model as well as consumer/survivor issues is required Demonstrated knowledge of Mental Health Act required Knowledge and understanding of diverse cultures required Excellent knowledge of housing, social, financial, work and other community resources Demonstrated ability to assess, plan, implement and evaluate client needs Excellent verbal and written communication skills required Demonstrated effective interpersonal and coaching skills required Flexible, organized and self-directed within scope of responsibility; has initiative and independent
judgement
