Program Summary

The Calgary Alternative Community Links (CACL) supports eligible adults with disabilities and complex support needs living in continuing care homes in Calgary. Through individualized, person-centered support, the program helps people build community connections, participate in meaningful activities, and access advocacy, referrals, and social inclusion supports that reflect their goals and needs.

Job Summary

The Community Support Worker III provides quality design and delivery of services within the Calgary Alternative Community Link (CACL) program. This role supports individuals in alignment with CAPCC program policy, emphasizing person-centered, community-based supports that promote inclusion, independence, and meaningful participation. The CSW III is responsible for:

• Ensuring that program content and service delivery align with CAPCC program policy, agency standards, and applicable funding requirements
• Supporting individualized, person-centered planning that reflects each client’s goals, strengths, and preferences
• Delivering services in a manner that promotes dignity, choice, inclusion, and community engagement

Key Responsibilities

Client

• Establish and maintain positive, professional relationships with clients, guardians, and support networks
• Provide direct support to an assigned caseload of clients in community-based settings
• Participate in the development and implementation of Individual Service Plans (ISPs) aligned with CAPCC program policy and individual outcomes
• Ensure supports are person-centered, culturally responsive, and promote autonomy and informed choice
• Collaborate with families, guardians, case managers, and other professionals to ensure coordinated service delivery
• Support clients in building and maintaining relationships within their communities
• Maintain accurate and timely documentation, including case notes within the CASS database
• Identify and connect clients to community resources that support their goals and inclusion

Agency

• Design and deliver services consistent with Calgary Alternative Community Link (CACL) program objectives
• Ensure all services align with CAPCC program policy, including accountability, documentation standards, and outcome-based service delivery
• Complete and submit all required documentation in a timely and accurate manner
• Participate in staff meetings and team collaboration as required
• Maintain all mandatory training and professional development requirements
• Support service quality by adhering to agency policies, procedures, and ethical standards
• Support the implementation and monitoring of Individual Service Plans and service agreements
• Ensure documentation meets CAPCC reporting and accountability standards
• Collaborate with internal and external stakeholders to ensure continuity of care and compliance with program expectations
• Communicate effectively with case managers and interdisciplinary team members regarding client progress and changes
• Work closely with the Community Support Coordinator/designate and team to provide regular updates regarding client needs, progress, and emerging concerns

Required Education

• Post secondary education in Human Services

Other Required Skills

• A current Basic First Aid/Emergency First Aid certification that includes CPR/AED
• A current Police Information Check with a Vulnerable Sectors Search that is satisfactory to CASS. Additionally, all employees have an ongoing duty to disclose any charges or convictions that may occur during their employment with CASS
• A vehicle and valid driver's license are considered assets
