Your Opportunity:
Primary Care Alberta is seeking a Health Promotion Facilitator II (HPFII), Quality and Process Improvement Specialist to join the Communicable Disease Control (CDC) Quality & Process Improvement team. Reporting to the Associate Manager, Quality and Process Improvement, this position leads the planning, implementation, monitoring and evaluation of quality improvement, process improvement and evaluation initiatives across the provincial CDC program. The role works collaboratively with internal teams and key partners to support process design, develop and implement improvement strategies, and facilitate change in a complex and evolving environment. The HPFII will lead the development of project plans including scope, methods, deliverables and timelines, and will guide teams in the creation of current and future state process maps, flowcharts, and other improvement tools. The position is responsible for identifying opportunities to enhance and streamline service delivery, supporting the design and implementation of measurement and evaluation plans, and monitoring progress toward defined outcomes. The incumbent will synthesize evidence and data from multiple sources, including literature and program data, to inform planning and decision making. This role requires strong skills in stakeholder engagement, facilitation, and communication, including leading discussions, presenting findings, and building consensus across diverse groups. The successful candidate will demonstrate expertise in quality improvement and evaluation methodologies, strong analytical and critical thinking skills, and the ability to manage multiple projects in a fast-paced environment. Proficiency with Microsoft Office tools and experience with data collection systems and survey platforms are required. This is a remote position that may require travel to a designated CDC site based on operational needs.
Description:
As a Health Promotion Facilitator II, you will lead and coordinate health promotion, disease and injury prevention initiatives and programs using a population health promotion approach. You will identify behavioural, social, political, economic, environmental and organizational factors that promote or compromise health and recommend evidence-informed strategies to address identified priorities. You will conduct situational analyses, literature reviews, environmental scans and focus groups to make recommendations as part of an integrated program planning and evaluation process. You will manage projects using standardized tools and techniques that are conducive or aligned with health promotion, disease and injury prevention practices. You will collaborate with key internal and external key stakeholders in project or program planning, development, implementation, evaluation and improvement. You will facilitate community and staff engagement and build community and staff capacity around population health promotion priorities. You will evaluate the effectiveness of existing health promotion and disease prevention strategies and make recommendations.

Required Qualifications:
Master's degree in Health Promotion, Public or Population Health, Health Education or a related field.
Additional Required Qualifications:
Minimum three (3) years' experience in a quality improvement, process improvement or evaluation role in a health care or public health setting, including leading substantial components of projects. Certification or formal training in quality improvement or evaluation methodologies such as Lean, Six Sigma, AIW, Institute for Healthcare Improvement or Canadian Evaluation Society. Experience applying quality improvement or evaluation methodologies such as Lean, Six Sigma, AIW or similar approaches. Strong analytical skills with experience collecting, managing and analyzing qualitative and quantitative data, including the use of databases and survey tools. Demonstrated ability to synthesize information from multiple sources into high quality reports, presentations and recommendations. Proven ability to lead projects, manage timelines and priorities, and work effectively in a fast-paced environment. Strong facilitation, communication and stakeholder engagement skills, with the ability to build relationships and consensus across diverse groups. Demonstrated critical thinking, problem solving and adaptability, with the ability to support change management and continuous improvement initiatives. Reliable internet access and ability to maintain a confidential workspace is required for remote work.
Preferred Qualifications:
Experience working in Communicable Disease Control or public health programs. Experience working within a large, complex health system. Experience using tools such as REDCap, SurveyMonkey or statistical software.
