Reporting to the Team Coordinator, PPDD, the Community Service Worker provides direct care and support to the individuals that receive funding through Disability Services/PDD in community settings within the Shared Living and Supportive Independent Living models. The supports provided promote well-being, independence, and community connectedness while assisting individuals to achieve goals and meet their personal outcomes.
Community Service Workers are required to provide physical, emotional, and/or behavioral support to persons with developmental disabilities in all areas of daily living, recreation/leisure activities and community inclusion. Client support includes personal care, lifting and transferring, behavioral intervention, assistance with meal preparation, and housekeeping activities. Community access will be required which will include swimming with clients.
- previous experience supporting persons with disabilities
- behavioural experience and ability to implement behavioural programs
- personal care experience and /or education
- previous community inclusion experience and knowledge of community resources
- strong verbal and written communication skills
- computer skills required
- ability to perform lifts and transfers per job requirements
Educational and other Qualifications
- Rehabilitation Practitioner Diploma or an equivalent combination of education and experience
- Valid First Aid and CPR Certification
Knowledge, Skills, and Abilities
- Comfortable using a computer including email and internet
- Strong interpersonal skills with an ability to deal effectively with conflict in a diplomatic and professional manner
- Adept at written and oral communication with internal and external customers
- Exhibits an open and sincere approach by demonstrating respect and compassion for everyone equally
- Maintains an individual’s dignity and self-worth
- Excellent active listening skills
- Demonstrates flexibility by changing routine to accommodate individual/resident needs
Work Experience
- Front line experience working with individuals with developmental disabilities
- Establishes and develops trusting and respectful relationship with the individuals receiving service
- Provides direct care and support based on individual’s needs such as: personal care, CPI, medication assistance
- Responsible for the day-to-day operations such as cleaning, grocery shopping, menu planning Ensures proper nutrition, manages materials and supplies, ensuring effective stewardship of financial and material resources
- Promotes an environment through role modeling that allows for choice, independence, skill development, and self-reliance for individuals/residents
- Implements behavioral management processes and completes accurate documentation
- Ensures the site is safe by implementing GSS safety administration practices, including but not limited to: fire drills, site maintenance, and completing documentation
- Ensures all documentation processes are completed
- Fosters and maintains open and receptive lines of communication with individuals/residents and their families/guardians, GSS, other service providers, and community professionals
- Responsible for completion and verification of personal timesheets
- Willingness to participate in committee and team meetings
- Responsible for complete and thorough orientation for new employees in the absence of a Team Coordinator
- Welcomes and introduces new individuals/residents into GSS services
- Demonstrates dependability by reporting to work on time and as scheduled
- Maintains a high standard of cleanliness, keeping the work environment consistently tidy
- Adheres to all infection control practices and procedures
- Ensures an environment and resources which nurture spirituality
- Other related duties as assigned
Requirements:
- A current and clear Criminal Records Check Form and if applicable a Vulnerable Sector Check that has been obtained no more than 90 calendar days prior to the hire date.
- A tuberculosis screening that has been completed no more than 12 months prior to the hire date.
