Support Office - Consultant, Home and Community Care Temporary Full Time – 38.75 hours per week (Approximately 1 year) Hours: 8:00 am – 4:15 pm with flexibility required, according to business needs Help Build the Future of Care – From the Ground Up What if you could design the next generation of home and community care? Covenant Living is looking for an experienced and visionary Consultant, Home and Community Care to lead the creation of an entirely new service line that will help bring high-quality, compassionate care directly into people’s home and communities. This isn’t a traditional consultant role. This is a rare opportunity to shape how care is delivered for years to come – designing everything from service models and care pathways to workforce strategies, digital solutions, implementation roadmaps, and hospital-to-home transitions. You’ll thrive in this role if you: Love turning big ideas into real-world solutions Get excited by complex challenges and system transformation Have deep expertise in home and community care Are equally comfortable discussing strategy with executives and workflows with front line teams Want your work to create meaningful impact for older adults, families, and communities What you’ll be doing You’ll lead the design of a scalable, implementation-ready model that brings together: Home care and hospital-to-home programs Clinical and operational workflows Care coordination and service delivery models Workforce and staffing strategies Digital health and virtual care solutions Quality, performance, and evaluation frameworks Partnerships across the health system In short: you’ll help create the blueprint for integrated, person-centered care beyond traditional facility walls.
