Position Summary
Physiotherapist: Mackenzie Health Outpatient Stroke Program
We are currently seeking a Physiotherapist to work in Stroke Outpatient Clinic located in Vaughan (955 Major Mac). Physiotherapist will provide patient management through all stages of assessment, treatment, consultation and follow-up services to patients. The successful candidate will be familiar with working in an outpatient clinic setting.
As a successful candidate, you are a graduate of an accredited program in Physiotherapy and are a member in good standing of the College of Physiotherapists of Ontario. You demonstrate excellent interpersonal, organizational and communication skills, both written and verbal, along with a high level of initiative and adaptability to rapidly changing demands.
Responsibilities
As a Physiotherapist (PT) you will be conducting assessments & treatment plans
As a PT you will be formulating goals and recommendations Planning and implementing patient centered, goal-directed treatment programs & working with Therapy Assistants Assist in the promotion of physiotherapy services to external referral sources Participating actively in the planning, implementation and evaluation of the program
Requirements
Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with a high-performance interdisciplinary rehabilitation team Graduate of an accredited program in Physiotherapy and are a member in good standing of the College of Physiotherapists Ontario 2 years of clinical experience in Stroke/Neurological patient populations is preferred Experience in assessment and generating treatment plans for patients with stroke/neurological disorders Knowledge of stroke best practices Experience with Chronic disease management is a good asset Sound knowledge of community resources is an asset NDT training is an asset Effective interpersonal, organizational and communication skills Strong customer service focus and excellent problem-solving skills required A positive work and attendance record is essential Ability to adapt to new computer applications CPR Level C + AED, and Standard/ Intermediate First Aide certificate Valid G license required Criminal background check, Vulnerable sector check and Immunization record (current)
