Overview

Intake Workers are key frontline members of SEOCHC, supporting individuals and families to access coordinated, wraparound health and social services. The role includes intake, triage, care coordination, brief counselling, and system navigation, with a focus on reducing barriers and improving equitable access to care.

Using a trauma-informed, anti-oppressive, and equity-focused approach, Intake Workers support priority populations impacted by the social determinants of health. The role helps clients navigate complex systems, understand their rights, and access services that reflect their goals, strengths, and lived experiences.

Working collaboratively with internal teams and community partners, Intake Workers contribute to coordinated, culturally safe care and support social prescribing initiatives that connect clients to community-based resources and supports.

The role also delivers two harm reduction programs and provides point-of-care HIV testing, including pre- and post-test counselling, supporting safer substance use, reducing risk, and connecting clients to relevant health and community services.

Services are delivered across clinic, community, and virtual settings to meet clients where they are and support continuity of care.

Key Responsibilities

• Conduct client intake, psychosocial assessment, and triage, ensuring timely access to appropriate services.

• Provide care coordination and brief interventions as part of an interdisciplinary team, supporting client-centred goals.

• Deliver therapeutic listening, system navigation, and referrals within scope of practice.

• Deliver harm reduction programming and provide point-of-care HIV testing, including pre- and post-test counselling.

• Identify and respond to risk, including crisis situations, safety concerns, and mandatory reporting obligations.

• Support clients to access income supports, community services, and social networks to promote stability and independence.

• Promote client rights, informed consent, and confidentiality, including communication of limits of confidentiality.

• Deliver services across clinic, community, and virtual settings to ensure accessible, continuous care.

• Build and maintain relationships with community partners to support coordinated care and referrals.

• Contribute to social prescribing and connect clients to community-based supports.

• Maintain accurate, timely documentation in alignment with organizational policies, privacy legislation, and standards of the Ontario College of Social Workers and Social Service Workers.

• Participate in supervision, team meetings, and ongoing professional development.

• Support outreach and represent the organization in community engagement activities.

• Must be able to travel between sites and work occasional evenings and weekends

Qualifications

• Bachelor of Social Work (BSW) or equivalent education with three (3)+ years’ related experience or an equivalent combination of education and experience in intake, counselling, and/or community-based social services.

• Experience working with priority populations using trauma-informed, anti-oppressive, equity-focused, and poverty-aware approaches; experience working with interpreters is an asset.

• Experience in psychosocial assessment, intake and triage, care coordination, system navigation, advocacy, and outreach.

• Knowledge of community resources and the social determinants of health, with the ability to build effective partnerships with service providers.

• Ability to assess and respond to risk, apply sound professional judgment, and support clients with complex needs safely and ethically.

• Ability to work both independently and within an interdisciplinary team in a fast-paced environment.

• Strong organizational and time management skills, with the ability to manage a high-volume caseload.

• Excellent verbal and written communication skills; fluency in English and French is required, additional languages considered an asset.

• Proficiency with computer systems and experience using electronic client records/EMRs.

• Commitment to ongoing learning and ethical practice aligned with standards of the Ontario College of Social Workers and Social Service Workers.

• Ability to work flexible hours, including evenings and weekends, and across clinic, community, and virtual settings.

• Satisfactory Police Record Check for the Vulnerable Sector required.

• Registration with the Ontario College of Social Workers and Social Service Workers, or eligibility for registration, is preferred.

• Must show proof of double vaccination for Covid-19. IS THIS STILL REQUIRED?

Salary: $53,435 to $62,881 per year
