The Calgary Foothills Primary Care Network (PCN) is a group of more than 600 family doctors and nurse practitioners who work with teams of other health professionals to offer patients the best in primary health care. Our PCN serves 400,000 patients in northwest Calgary and Cochrane. Together with our community partners, we offer round-the-clock care. We work to achieve better health outcomes for our patients. The Mental Health Consultant will provide patient-centered, single-session counseling and mental health support within an interdisciplinary primary care team. Responsibilities include conducting individual and group appointments, evaluating crisis situations, and collaborating with healthcare providers to navigate patient care.

The Clinical Pharmacist manages medications, conducts reviews, and provides evidence-based therapeutic information within a multidisciplinary primary care team. They also collaborate with patients to develop care plans, provide coaching for chronic disease management, and facilitate group-based health programs.

The Indigenous Lead acts as a trusted advisor to foster respectful relationships between Indigenous communities and the PCN while advancing cultural humility. This role involves providing strategic advice to leadership, supporting health equity initiatives, and coordinating with provincial Indigenous health programs.
