Champlain Rehab Solutions emerged from the integration of COTA Health, a not-for-profit rehabilitation services organization founded in 1973, and VHA Home HeathCare (VHA), a not-for-profit nursing, personal support, and community support services agency established in 1925. Our Champlain Rehab Solutions division in Ottawa provides physiotherapy, occupational therapy and social work services for clients to achieve the best outcomes possible. We offer high-quality, multilingual services in Ottawa and our work environment focuses on supporting clients, their families, and service providers through client-centred care, well-connected health-care teams and continual improvement. Most of our rehabilitation services are provided free of charge through our contract with the Champlain Home and Community Care Support Services (HCCSS) and the Ottawa and Ottawa East Ontario Health Teams. The Physiotherapist provides consultations, assessments, and planned interventions for diverse client groups in home and community settings. They are responsible for coordinating care with integrated teams and documenting all treatment progress and discharge summaries.
