Housing Support Worker (Midnights)

On-site · Leamington

Support people experiencing homelessness in accessing and maintaining housing through assessments, individualized housing plans, advocacy, referrals, and assistance with applications and financial supports. Conduct outreach across Essex County, assess risks, monitor client progress, and coordinate services with community partners while following program and regulatory requirements.

Voluntary Trusteeship Program Worker

The worker will conduct eligibility assessments, develop individualized service plans, and provide direct support to help clients maintain independence and access community resources. They will also advocate for client rights and coordinate with various community services, healthcare providers, and government agencies.

Housing Outreach Worker

On-site · Windsor

Conduct outreach to identify people experiencing homelessness, assess their housing needs, and connect them with appropriate resources and coordinated access services. Find and secure housing, support clients with the steps needed to establish a home, coordinate warm transfers to other programs, and document activities.

Affiliate Counsellor

On-site · Windsor

Provide short-term, solution-focused counselling and EAP clinical support to diverse client groups. Conduct thorough assessments, develop goal-oriented treatment plans, and maintain accurate, confidential client documentation.
