Position Summary:

A hands-on community health position where collaboration with the formal and informal care networks supports a client and family-centered home care model in your local neighborhood. The Regulated Health Professional team lead builds a collaborative team based on the principles of trust, autonomy, creativity, and simplicity. The personalized approach to home care is curated for people who wish to age in place their way. The Clinical Leader builds relationships in the community, provides direct care, within their scope of practice, in client homes, and supervises their team. The clinical leader will support their client with alternative solutions in the event of staff absence when no replacement is available.

Problem Solving:

  • This role requires critical thinking and problem-solving related to at-home care.

    Typical Problems Solved:

    • Ensuring an appropriate fit between members of the team and your client.

    • Ensuring shift coverage for sick calls.

    • Managing expectations with family members and clients and ensuring a client and family-centered approach to care planning.

    • Adapting care to changing client needs, including occasionally working with the interdisciplinary team for management of acute illness, falls, and other immediate challenges.

    Decision Making

  • Typical Decisions Made:

    • Financial decisions concerning bill rates using a decision tree and rate card structure
    • Hiring decisions for Personal Support Workers, companions, and other Regulated Health Professional team members
    • Decisions around the ability to create and implement a client and family-centered care plan
    • Service decisions based on clear and concise program criteria
    • Escalation to physician or other members of the care team based on client condition and judgment

    Management Responsibilities:

  • Management responsibility for the field staff team members with Director of Growth and Development support.

Communication:

  • External: Medical services in the community, such as physicians, nurses, social workers, other HCP’s, discharge planning Regulated Health Professionals, pharmacists, and medical supply companies, LTC community members, clinics, and family health teams and other relevant local community connections.

Physical Requirements:

  • Ability to travel to various locations, walking standing sitting for long periods. Ability to lift more than 20lbs, ability to physically carry out direct patient care, potential for workplace violence during conflict resolution or working with clients who have dementia. Some computer work requiring prolonged exposure to blue light, repetitive motion (mouse, typing,)

Client Care:

  • Ensure holistic assessments and ongoing reassessments for positive outcomes.
  • Develop client-centered care plans
  • Visit clients as needed to manage care, services, and monitor client/health care provider (HCP) matching.
  • Investigate, document and follow up promptly on complaints from clients, client families, and/or funders
  • Participate in business development and community engagement activities.

Human Resources Coordination: Supervision and Support of Field Employees (Professional and/or Paraprofessional):

  • Participate in recruitment and selection of field employees
  • Promote and improve team engagement. Conduct support visits and performance reviews of field employees.
  • Ensure compliance with federal and provincial standards and company policies.
  • Investigate and document employee complaints/concerns while maintaining unbiased and confidential practices
  • Provide ongoing feedback and performance management including: coaching, disciplinary actions and documentation.
  • Facilitate a learning environment to optimize patient outcomes; lead/provide training, support, and education Interpret and implement relevant policies and procedures.
  • Mentor clinical team members and foster the mentorship framework.

Interdisciplinary Liaison:

  • Coordinate with funders, government programs, and care team members.
  • Implement, monitor, and evaluate delegated tasks to unregulated care providers.
  • Attend and participate in client case conferences

Education and Professional Development:

  • Attend and share knowledge from internal and external education offerings.
  • Engage in ongoing professional development and continuing education activities.
  • Provide mentorship to other managers and administrative team members.
  • Develop and present training plans for employees and deliver company developed in-services education

Health and Safety:

  • Maintain privacy and confidentiality of client and corporate information, including appropriate digital/cybersecurity safeguards.
  • Ensure compliance with health and safety regulations and company policies.
  • Promote a safe working environment for all employees and clients.
  • Lead or assist with the implementation of Modified Work Programs for field employees.
  • Implement and monitor health and safety protocols to prevent workplace injuries and illnesses.

Support Branch Operations:

  • Manage branch operations in the absence of senior management when needed.
  • Ensure 24/7 clinician access and serve as on-call nurse/clinician as needed.
  • Use electronic health records to improve clinical outcomes and practice standards.
  • Additional Tasks: Complete other tasks as requested.

Qualifications:

Education & Training:

  • Graduated from an accredited healthcare program.
  • Valid professional license and current registration with the provincial regulating body
  • Valid driver's license, and a clear driving abstract and access a reliable personal vehicle

Experience:

  • At least five years of experience with progressive responsibilities.
  • People management experience.
  • Experience working with adult and geriatric clients and their families.
  • Experience working with children an asset.
  • Experience with community connections and relationship management.
