The Opportunity!
In coordination with the Chief Operating Officer, Director of Clinical Services and Manager of DS Options program, the Project Coordinator for the DS Case Management Program will be responsible for overseeing the successful planning, execution, and completion of program implementation. This role involves working closely with partners, managing data and process flows, training staff, and ensuring that project timelines are met.
Responsibilities:
- Collaborate with internal and external partners to ensure alignment and effective communication.
- Manage and analyze program data to support decision-making and reporting.
- Oversee and optimize process flows to enhance program efficiency and effectiveness.
- Train and support staff involved in the DS Case Management Program and tools being developed
- Monitor project progress and ensure that all timelines and milestones are met.
- Ensure the program meets the needs of clients, service partners and funders.
- Report regularly to the COO on project status, risks, and outcomes.
Qualifications:
- Bachelor’s degree in a relevant field (e.g., Social Work, Public Administration, Project Management).
- Proven experience in project management, preferably in a social services or healthcare setting.
- Strong data management and analytical skills.
- Excellent communication and interpersonal skills.
- Ability to lead and train staff effectively.
- Familiarity with the Toronto region and its community services is an asset.
