About the Role
We’re currently hiring a Social Worker – Short Term Case Management with a strong background in elder support and case management. This role focuses on providing short-term, goal-oriented services to older adults, with a special focus on housing support, system navigation, and elder abuse intervention. You’ll also have the opportunity to mentor Bachelor of Social Work (BSW) students, offering guidance grounded in trauma-informed practice.
What You’ll Be Doing
- Provide short-term, client-centered case management with empathy, efficiency, and professionalism.
- Assist older adults with housing applications, system navigation, and connection to relevant community supports.
- Support our Elder Abuse Intervention Program, including safety planning, advocacy, and crisis response.
- Supervise and mentor BSW students, integrating real-world experience with social work best practices.
- Maintain accurate and timely case documentation that meets agency and regulatory standards.
- Collaborate with community partners and stay up to date with local service options.
- Participate in team meetings, case consultations, and staff development opportunities.
- Approach client needs through a trauma-informed, person-centered lens.
- Contribute to broader agency goals, including service integration and continuous improvement.
What You Bring
- Bachelor or Master of Social Work from an accredited institution (required).
- Registered and in good standing with the Ontario College of Social Workers and Social Service Workers.
- Minimum of 5 years of front-line experience, ideally in community-based social work.
- Proven experience supporting older adults and understanding of aging-related issues.
- Knowledge of housing systems, subsidies, and supportive housing options.
- Understanding of elder abuse dynamics and intervention strategies.
- Strong organizational and time-management skills, with the ability to manage multiple cases.
- Experience supervising or mentoring students is an asset.
- Solid knowledge of local community resources and a collaborative, team-oriented approach.
- Valid driver’s license and access to a reliable vehicle.
Position Details
- Job Type: Full-time (35 hours/week)
- Schedule: Monday to Friday, with occasional evenings or weekends as needed
- Salary: Starting range based on experience and education $58,344-$66,300
- Location: Windsor-Essex
