About The Role Key Accountabilities
- Plan, coordinate, and implement the safe transition of new individuals into the service and community where necessary.
- Liaise with the families of the people we support and relevant members of multi-disciplinary teams, community groups, and voluntary bodies to promote their needs.
- Plan, develop, and implement individual and group programmes of support to enhance and maximize the capabilities and independence of the people we support in the service.
- Coordinate ongoing assessment and review regularly through the completion of suitable assessment models.
- Support the physical, emotional, and personal needs of the people we support, ensuring that work is individual-focused and upholds the principles of respect, privacy, dignity, fulfilment, independence, and choice.
- Ensure that medication is held, stored, and administered according to Praxis Care’s Medication Policy and current statutory and legislative procedures.
- Provide effective leadership, supervision, and management to Support Workers, including monitoring time and attendance and participating in any capability, disciplinary, and grievance procedures.
- Participate in learning and development as necessary within the organization while identifying and addressing personal development opportunities for themselves and Support Workers.
- Ensure that adequate records are created and maintained, and necessary reports are produced in line with organizational policy and statutory requirements.
- Support the manager with regulatory compliance concerning all regulating bodies.
Requirements Of The Role
- Participate in the company appraisal and staff review process.
- Adhere to and model the company values, behaviours, and competencies at all times.
- Engage in formal and informal supervision as required.
- Carry out all duties in accordance with Praxis expectations, regulations, and policies.
- Take on additional duties of the role and other required duties as necessary.
- Work the necessary hours to fit the needs of the service.
- Where the role requires sleep-ins, a minimum additional payment of €101.60 per night applies
The above-mentioned duties are not exhaustive, and the post holder may be required to carry out other duties as necessary. It should be noted that stated duties or locations associated with the post may change to meet the future needs of the organisation.
Essential And Desirable Criteria Education, Experience & Training Qualifications Essential
- 2 years’ experience in a health and social care setting and a minimum of QQI Level 5 in Health Care or equivalent.
Desirable
- Experience in mental health or learning disability services.
Skills & Competencies
- Full valid driving licence and access to own vehicle (If a disability precludes an individual from holding a driver’s licence, it should be stated how they can meet the mobility requirement of the post).
- Strong verbal and written communication skills.

- 2 years’ experience in a health and social care setting and a minimum of QQI Level 5 in Health Care or equivalent.
