About the role

We are seeking a passionate and organized Outcome Support Supervisor to lead and enhance the integration Personal Outcome Measures (POM) throughout the Guelph Wellington area. This permanent full-time leadership role supports our organizational direction and ensures a consistent, high-quality approach to person-centred services across the organization.

As an Outcome Support Supervisor, you will be reporting to the Certified Personal Outcome Measures Trainer within Community Living Guelph Wellington, and you will be collaborating across departments to embed POM practices into everyday service delivery.

Your Responsibilities:

  • Lead the implementation and integration of POM across all CLGW services and processes.

  • Conduct a minimum of 50 POM interviews annually, ensuring quality and consistency in data collection.

  • Deliver onboarding and POM training for staff, including two 4-day workshops per year.

  • Support, conduct inter-rater reliability and coordinate DSPs trained in POM interviews; provide mentorship and technical assistance.

  • Lead internal committees, including the development and execution of a Right Restrictions Committee and the POM Resource Team.

  • Facilitate Lunch and Learn sessions and create in-house and social media content focused on person-centred practices.

  • Analyze and interpret POM data to identify trends; make recommendations for training, processes and policy development, and support continuous service improvement.

  • Meet and collaborate with individuals supported, families, and staff to ensure that services align with personal goals and outcomes.

  • Provide support to ensure the implementation of recommendations as it relates to the goals of the person supported

  • Participate in the Tier 1 on-call rotation, working on-call for a week, approximately once every 12 weeks.

  • Available to work Monday to Friday, however, must be able to adjust your work schedule depending on the needs of the people you are supporting.

You Have:

  • Certification or willingness to become certified as a POM Trainer

  • Post-secondary education in social services, disability studies, or a related field.

  • Minimum 3 years of experience in developmental services or a related human services field.

  • Strong understanding of person-centred planning and outcome measurement tools.

  • Proven facilitation and training skills.

  • Excellent communication, analytical, and organizational abilities.

  • Demonstrated commitment to inclusion, equity, and continuous improvement.

  • Preferred Assets:

  • Experience with the framework or direct knowledge of POM.

  • Familiarity with data analysis and quality improvement strategies.

  • Lived experience with developmental disabilities (self or family member) is considered an asset.
