The Clinical Manager, Outpatient Services is currently seeking a full-time Community Clinician for the Outpatients Assessment and Treatment services program. To provide nursing and mental health services to individuals with a serious mental illness living in a community setting and/or preparing to live in the community, may use intensive/assertive community treatment and psychosocial approaches to reintegrate into the community and nursing expertise and leadership within the multidisciplinary team: as well as to provide liaison, education and consultation services between long term care facilities, group homes, community agencies, boards and committees. Position provides nursing expertise in the provision, coordination, and monitoring of complex psychopharmacological interventions.
Please Note: This position offers a hybrid work environment consisting of in-office and remote work. The office location is 287 Bayshore street, Midland ON.
KEY ACCOUNTABILITIES:
- Comprehensive mental health and psychosocial assessments, documentation including evidence based treatment and responses, assessment of activities of daily living and general health issues, interview and observe clients, family, other service providers
- Work collaboratively with the client, multidisciplinary team and community partners to develop, implement and evaluate the client’s recovery plan of care. Communicate with stakeholders regarding client’s progress and any specific concerns related to wellness.
- Develop, implement, and evaluate therapeutic treatment strategies and crisis management strategies with clients, families, other service providers.
- Support client rehabilitation and community integration in collaboration with community partners and primary care providers
- Liaise with community agencies/facilities to meet the needs of the population being served, provide information and promote and advocate for community based programs and services. Provide health teaching and knowledge translation as appropriate.
- Develop, implement, facilitate and evaluate different treatment modalities i.e. group and individual therapies for example, Cognitive Behavioural Therapy (CBT), Dialectical Behaviour Therapy (DBT) and other evidenced based therapies as appropriate. Participate in data collection, research and program evaluation projects; participation on committees
REQUIREMENTS/QUALIFICATIONS:
- Registered Nurse in good standing with the College of Nurses of Ontario
- Valid certification in Basic Life Support (BLS) with Oxygen Therapy (OT) and Airway Management (AM)
- Valid Ontario Driver’s License
- Recent mental health experience, with preference for community experience in a mental health facility or service, including familiarity with the recovery philosophy, and trauma informed care
- Experience developing and leading therapeutic groups and providing individual therapies such as CBT and DBT an asset.
- Advanced knowledge of and experience with the theories and best practices of your regulated profession(related to mental health illness/addiction)
- Proven experience with evidence based assessment,treatment, interventions and best practices
- Results oriented work ethic to work independently and undertake tasks needed to accomplish work objectives and deliver quality, consistent and timely results
- Demonstrated leadership, analytical thinking, conflict resolution, interpersonal and organizational skills
- Proven written and verbal communication skills
- Proven ability to adapt readily to change
- Models and promotes core ethical practice, Waypoint Values (Caring, Respect, Accountability, Innovation), and reflects an optimistic and positive attitude
- Preference for proficiency in French/English language skills
