Job Description
The Integrated Care Team (ICT) is a multi-disciplinary team that provides comprehensive care to individuals experiencing homelessness and complex health issues in the Downtown Eastside (DTES) of Vancouver. The Social Worker will work collaboratively with other team members to provide assessment, intervention, and support to clients.
Main Responsibilities
- Conduct comprehensive assessments of clients’ needs and strengths.
- Develop and implement individualized care plans.
- Provide counseling and support to clients and their families.
- Collaborate with community resources and services to facilitate client access.
- Participate in team meetings and case conferences.
Required Qualifications
- Master’s degree in Social Work from an accredited institution.
- Registration with the British Columbia College of Social Workers.
- Minimum of 2 years of experience in a social work role, preferably in a community setting.
Preferred Qualifications
- Experience working with marginalized populations.
- Knowledge of harm reduction principles.
- Strong communication and interpersonal skills.
Other Relevant Details
- This position requires a valid driver’s license and access to a vehicle.
