What we are looking for:

As a Clinical Dietitian (CD), you will become an integral member of the Community Health Clinic (CHC) operated in partnership with Oak Valley Health and the Eastern York Region North Durham Ontario Health Team (EYRND OHT).

What you bring to the role:

As a member of the interprofessional primary care team at the CHC, you will work closely with Nurse Practitioners, Primary Care Physicians, and other Allied Health professionals. This new role is focused on providing clinical assessments, interventions, recommendations and maximizing the scope of the CD in a primary care setting. This role will also contribute to the creation of integrated clinical pathways within the EYRND OHT. You will also have the opportunity to participate in the ongoing program development and quality improvement activities.

Requirements:

• Current membership in the College of Dietitians of Ontario, in good standing
• Membership with Dietitians of Canada (DC) preferred
• CNSC and/or CDE designation is an asset
• Minimum 1 year experience is preferred
• Demonstrated commitment to ongoing professional development and evidence-based practice
• Excellent assessment, organizational, and interpersonal skills.
• Demonstrated excellence in verbal and written communication skills
• Demonstrated competency with electronic medical record charting systems and associated computer skills
• Demonstrated good attendance and performance records with the ability to maintain these same standards
• Demonstrates initiative and the ability to function both independently and as a multidisciplinary team member
• Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required

Work Schedule:

Successful applicants should be available to work Monday through Friday 8:30 a.m. - 4:30 p.m., with occasional evening and weekend hours as required.
