Your impact

  • Deliver a personalized, inclusive and elevated care experience in a collaborative and a multidisciplinary environment
  • Bring strong verbal and written communications that allow patients to feel heard and supported

Your responsibilities

  • Coordinate the physician’s and nurse practitioner's schedules including their clinic
  • Communicate with patients via phone, email, EMR portal, and in person on a regular basis
  • Educate and support patients around clinic processes, diagnostic testing, and treatment
  • Document all communications with patients appropriately and thoroughly
  • Perform data entry as necessary
  • Schedule diagnostic tests for patients
  • Assist with inbound and outbound referrals management if required
  • Communicate with other physician and allied health care provider offices as needed
  • Help shape the care coordinator role by identifying opportunities to drive efficiency and a better experience for our clinic and patients

Your experience

  • Previous related experience in a medical or clinical office environment
  • Successful completion of a post-secondary certificate or diploma in Medical Administration or equivalent (preferred)
  • Strong interpersonal skills and a high degree of discretion
  • Effective verbal and written communication skills
  • Must enjoy learning and interacting with people of all backgrounds
  • Experience, willingness and comfort with providing care to LGBT2SQ individuals and couples
  • Comfortable working in a fast-paced environment and adaptable to change
  • Excellent time management abilities and attention to detail
  • Highly effective organizational skills, flexibility, and adaptability
  • Proficient in Microsoft Office or equivalent
  • Experience working within an EMR and/or CRM (preferred)
