Reporting to the Manager, Community Health and Well-being and working as part of an interdisciplinary team, the Public Health Dietitian is to deliver community-based nutrition promotion programs and services and to support community-initiated food and nutrition projects/initiative.
Major Responsibilities:

Provides public health dietetic services for individuals, families, groups, schools and the community according to program specific policies and procedures, operational plans of Toronto Public Health (TPH) and the standards of the College of Dietitians of Ontario.

Conducts relevant screening and assesses nutritional needs at the individual/family/school/community level and develops appropriate action plans and work plans to meet those needs, using a consultative approach, in collaboration with other TPH streams, directorates and City of Toronto divisions and community partners, as appropriate.

Delivers appropriate nutrition interventions from a health promotion and behavior change perspective, based on the nutrition needs assessment conducted for the individual/family/school/community.

Facilitates and/or participates in community partnerships and collaborative initiatives and represents TPH on assigned networks/committees.

Utilizes a variety of health promotion strategies such as advocacy for healthy public policies, community capacity building, creating supportive environments and education/skill building.
-
Assesses and makes referrals to other TPH services, other health professionals/agencies and directs clients to community resources; liaises with other program areas and community agencies to ensure communication of relevant information and issues.

Participates in the planning, development, implementation and evaluation nutrition programs/services, projects/ initiatives, using a population health approach to address both citywide and local/regional food/nutrition needs.

Participates in coordination of projects as determined by program specific operational plans and work plans and prepares resources and website content for diverse communities and groups.

Provides support to community-initiated food/nutrition programs

Maintains service delivery records according to policies and procedures of TPH as well as the Standards of Practice set out by the College of Dietitians of Ontario.

Prepares, submits and presents activity reports, data and other reports, such as time and activity data collection, program statistics and documentation.

Mentors and provides training, guidance and support internally to staff and students and externally to staff, volunteers and leaders in schools and community agencies/organizations.
-
Presents and participates in professional development activities and delivers presentations to colleagues and other health professionals.
-
Participates in the development and implementation of policies, procedures, operational plans, work plans, and updating of resources pertinent to the assigned program area.

Participates in applied nutrition research.
Key Qualifications:

Membership with the College of Dietitians of Ontario.

Experience working with a variety of groups in a culturally diverse community.

Possession of a valid Class "G" Ontario Driver's License and access to a vehicle.
You must also have:

Knowledge of population health approaches, client engagement principles and health promotion theory.

Excellent oral and written communication skills.

Excellent assessment, consultation, and group facilitation skills.

Excellent interpersonal skills with the ability to establish and maintain good public relations.

Ability to work using anti-oppressive practices and harm reduction approaches.

Ability to provide services appropriate to clients and communities with diverse cultures, health beliefs and practices.

Ability to work effectively in interdisciplinary teams/groups and collaboratively with internal and external partners
-
Ability to work flexible hours - overtime, shift work including evenings and weekends may be required.

Ability to travel within city, to schools and community sites
