The Ministry of Social Services is committed to positive outcomes in the area of support for persons with disabilities. Community Living Service Delivery is seeking a highly motivated and task-oriented individual to fill the position of Outreach Nurse who is passionate about supporting and assisting intellectually disabled individuals to live their lives to the fullest. As part of Clinical Crisis Services, you will be a part of a dynamic team that includes Crisis Support Therapists, Program Development Consultants, Outreach Workers, Occupational Therapists, Physio Therapists, Recreational Therapists and Therapy support staff. You will be responsible for the development and the delivery of nursing support as well as play an active role in health education and identifying ways to improve mental/physical health care services to a variety of stakeholders including government services, service providers, families, and community-based organizations.
Our candidate of choice will have:
- the knowledge and skills gained and demonstrated through a bachelor's degree in Nursing, and is currently (or eligible to be) registered with the RPNAS or SRNA;
- several years of post-academic nursing experience including at least two years of experience working with individuals with developmental and physical disabilities, including living within a community setting; and
- experience of development and/or delivery of therapeutic, educative, clinical services within the mental health field and the array of services available to individuals.
You will have the ability to provide reactive and proactive solutions to address physical and mental health challenges to enhance the lives of individuals with disabilities. You will be required to reconcile divergent views and interests and work towards mutually acceptable solutions with a strong ethical and person-centered approach.
This position may be exposed to the risk of violent situations. Travel is a requirement of the position, including overnight stays.

The schedule for this position is Monday to Friday 9am-5pm with every third Friday off.
