Your Opportunity:
The Administrative Support IV - Flow Team Coordinator will be an integrated member of the AMH Supported Housing Flow Team. The coordinator will provide extensive day to day system support for the Strata Pathways referral system utilized by Addictions & Mental Health Supported Housing. The coordinator will function as the primary point of contact for information, education and support, and will be responsible for triaging and escalating requests requiring clinical response. This individual will also be responsible for information management, data monitoring and reporting. The coordinator will have an excellent working knowledge of the Strata Health Care Pathways referral system, a strong understanding of the AMH Supported Housing continuum, and strong data analysis and reporting skills.

Required Qualifications:
Some post-secondary education.

Completion of post-secondary education (e.g. course(s) in business administration, medical office assistant), or other related fields. Experience utilizing MS Office suite, including Excel. Proficient with Connect Care. Some knowledge/awareness of AMH Supported Housing and Strata Pathways.
Preferred Qualifications:
Experience working within addiction and mental health.
