About us:

Founded in 1960, the Youth Services Bureau of Ottawa (YSB) is one of the largest and most comprehensive non-profit agencies in Ottawa and a registered charity. It provides mental health services, employment programs, emergency shelter, community, health and housing services, and justice programs to youth in the Ottawa area, serving between 2,500 and 3,000 at‑risk individuals aged 12+ and their families every month. YSB employs 400 caring professionals in a unionized environment in over 21 locations across Ottawa.

Reporting to the Coordinator, Mental Health, the Care Coordination worker is responsible for facilitating client access to services, monitoring program outcomes and actively contributing to the effective administrative functioning and operations of the program.

Principal Duties:

  • Awareness of developments/changes in the community pertaining to the service system and client needs
  • Review client data to determine eligibility for referral
  • Collaborate with referring agency to work towards resolution and ensure most efficient/appropriate referral process is accessed
  • Prepare and facilitate case planning meetings
  • Determine/Invite appropriate members for case planning meetings
  • Develop client service plans
  • Present cases to the resolution committees and ensure on-going liaison with them
  • Promote collaboration between various service providers in order to develop flexible and creative service plans
  • Promote philosophy of access and resolution functions to agencies and the community at large
  • Support One Call/One Click activities; including providing short term case coordination and brief therapeutic support
  • Advocate for access to services on client’s behalf
  • Monitor flex funds expenditures
  • Develop / revise / review and implement necessary program monitoring system
  • Monitor service plan implementation and outcome
  • Conduct follow-ups in accordance with the program policy
  • Monitor program data-base
  • Analyze program evaluation feedback and outcome data to develop reports
  • Ensures both electronic and physical client files are up to date, maintained and handled within established practices
  • Follow safe work practices as established by the Bureau and the Program

Job Specifications / Qualifications

  • Master’s degree in Social Work/Psychology or relevant field preferred
  • Bachelor’s degree in Social Work/Psychology or relevant field, equivalency may be considered
  • Must be registered and in good standing with a college
  • Minimum 5 years work experience in relevant Social or Mental Health related discipline
  • Knowledge of local community and social service system
  • Demonstrates decision making and problem-solving skills
  • Proven organizational and administrative skills
  • Demonstrates strong interpersonal skills and collaborative abilities
  • Strong written and verbal proficiency in English and French mandatory.
  • Experience in group facilitation
  • Broad cross-sectoral experience would be an asset

Additional Qualifications:

  • Successful applicants will be required to provide a clear Criminal Record Check, Vulnerable Sector Check or Broad Police check obtained within the past six (6) months, as a condition of employment with YSB.
  • To confirm language proficiency required for this position, successful candidates may be requested to undergo an oral and written assessment conducted by a third-party provider, arranged by YSB. Candidates must meet the minimum required linguistic profile before their employment start date. Formal education may be considered as sufficient demonstration of language proficiency, but is subject to review.
