About the role

Community Ambassador – Inter Professional Care Team

1 FTE Contract to March 31, 2027

Site Location -Outreach

One of a network of provincial community health centres, the NorWest Community Health Centres is a community-based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.

We are currently seeking one full-time Community Ambassador who is passionate and committed individual who will a key member of our Inter Professional Care Team. This role is dedicated to building strong community connections withing District Social Service buildings; fostering trust, promoting programs and services, and supporting vulnerable and marginalized populations in accessing care.

The position has a competitive salary, excellent group health benefits and the option to join the Health Care of Ontario Pension Plan (HOOPP).

QUALIFICATIONS:

  • Secondary school diploma, or equivalent work experience.
  • Experience working in community outreach, peer navigation, or health promotion roles with all genders, particularly with older adults, Black/Indigenous, newcomer, 2SLGBTQIA+ communities, persons experience mental health or substance use, or involvement in the criminal justice system.
  • Lived experience related to community health, mental health and addictions, or navigating systems of care as a client or family member.
  • Comfort facilitating group discussions, outreach activities, and one-on-one support in community settings.
  • Ability to build trust and establish supportive, culturally safe relationships with community members.
  • Demonstrated ability to be flexible, organized, a self-starter, and possess excellent problem-solving skills.
  • Ability to speak languages other than English considered a strong asset.
  • Proficiency in the use of computers and Microsoft Office.
  • Valid driver’s license or ability to travel to local sites and events is required.
