Overview The Coordinator, Resident Assessment Instrument (RAI) is responsible for overseeing the RAI process within a Long-Term Care Facility, ensuring that all resident assessments are completed accurately and in a timely manner in compliance with provincial and federal regulations. This role involves collaborating with clinical staff to assess residents’ care needs and develop care plans that align with the RAI results. The Coordinator also ensures that documentation is maintained, and reports are submitted to the Ministry of Health and Long-Term Care in accordance with regulatory requirements.
- Coordinate and oversee the administration of RAI assessments for all residents within the facility, ensuring that assessments are completed thoroughly and in compliance with regulatory standards.
- Oversee the collection, entry, and management of Resident Assessment Instrument data into the electronic medical record system, ensuring accuracy and compliance with provincial regulations.
- Ensure adherence to Ministry of Long-Term Care standards and Resident Assessment Instrument guidelines and support the implementation of continuous quality improvement initiatives related to RAI assessments.
- Provide guidance, training and ongoing education to staff regarding RAI procedures, coding, and the integration of assessment data into care planning.
- Assist in preparing for regulatory audits, ensuring that all relevant RAI documentation is complete, up-to-date, and compliant with regulations.
- Analyze RAI data to identify trends, risks, and opportunities for care improvements and report findings to senior management and regulatory bodies.
Position Requirements
- Bachelor’s degree or diploma or other equivalent certification in Nursing, or Social Work, or a related healthcare field.
- RAI-MDS certification
- Minimum of 2 years of experience in long-term care, with a focus on RAI assessments, data management, or quality improvement.
Infection prevention and control requirement, as outlined by the O. Reg. 246/22: General under the Fixing long-term care Act, 2021, S.O. 2021, c 39, Sched. 1:
prospective candidates will need to demonstrate screening for tuberculosis and other infectious diseases in accordance with any standard or protocol issued by the Director. This includes DTAP, MMR, Varciella, and Hepatitis B.
Schedule: 35.00-hour/week, schedule may vary based on operational requirements.
