Job Title: Case Manager (HART Hub)
Job Type: Four (4) Full Time Unionized position (35 Hours/Week: Monday to Friday)
Language: Two (2) Bilingualism (French and English). Two (2) English required.

Job Description:
The Case Manager provides community-based support to individuals suffering from severe and persistent mental illness. Client needs are assessed, a service plan is developed and the client is referred to appropriate community resources. The service is flexible, portable, individualized and client-centered. The Case Manager provides assistance in the following life domains: basic needs, daily living, physical and mental health, housing, financial, social, spiritual, legal, educational and vocational.
This position may require working in an environment with exposure to challenging conditions, including unpleasant situations and odors. The successful candidate must have a demonstrated understanding of mental health, substance use and harm reduction principles to provide inclusive support for the population in the building.
Qualifications:
- Bachelor’s degree in Social or Health services, related field or equivalent
- At least three years of experience working with adults who have a psychiatric disability
- Suicide Prevention Training
- Non-Violent Crisis Intervention
- Knowledge of the following:
- Community mental health resources in Ottawa
- Acute symptoms and residual effects of major mental illness
- Housing, financial, mental health, physical health and other community resources and the referral procedures
- Interpersonal, family, and organizational relationship dynamics
- Principles of psycho-social rehabilitation and case management
- Concurrent disorders and related best practices
- Knowledge of the challenges surrounding homelessness and forensic mental health
Suitability Profile:
- Establishes rapport with a wide variety of people
- Supports the notion of independence through the least amount of professional intervention and orient to the notion of closure
- Utilizes a client-centred approach to case management
- Advocates for individual needs and program changes
- Assesses and prioritizes needs
- Supports the development of an Early Intervention Plan
- Coordinates service delivery
- Identifies service delivery gaps
- Works both collaboratively and independently
- Engages difficult clients in creative and innovative ways
Pre-Hiring Requirements:
- Academic Certificates
- Criminal reference check (Vulnerable Sector Check)
- Eligibility to work in Canada
- Valid drivers’ license and use of own vehicle with appropriate insurance is required.
- Proof that the successful candidate has received both doses of a Health Canada approved COVID 19 vaccine at least 14 days before the first day of work, subject always to Ottawa Salus’ duty to accommodate under the Ontario Human Rights Code.
