Job Description:
The Patient Flow program at Trillium Health Partners is looking for an experienced, energetic professional to lead our project teams on a journey into the next phase of delivering our vision at THP. As the Flow Project Manager, this individual will be a core member of the project team (Process and System Integration) that will lead projects and improvement cycles across the patient flow platform across multiple sites. The successful candidate will be a part of a project team reporting to the Director, Access, Flow, Operations and Process and System Integration.
You are an individual who wants to play an integral role in transforming the way we care for patients of our community. You enjoy thinking creatively about how to solve problems and unlock opportunities related to process and structure. You enjoy working collaboratively, are curious, creative and persistent, with a thirst for learning and a natural aptitude for driving change. You are experienced with leading teams through projects using a framework and enjoy coaching others to grow skills and knowledge. You are passionate about supporting care that honours our values of excellence, compassion and courage.
The project manager will focus on transforming current practices through the flow portfolio in the areas of:
- Optimization of Capacity Management
- Integrated process design for care transitions and patient flow
- Standardization of bed management and surge space utilization
- Data analysis and flow performances
Responsibilities:
- Guide all aspects of the project/improvement (e.g., quality risk, communication, staffing, etc) and provide project specific vision, goals and objectives to the involved teams and key stakeholders.
- Provide leadership in the development and implementation of all aspects of assigned projects and improvement cycles
- Identify, track and report process metrics and key performance indicators
- Facilitate meetings and stakeholder engagement sessions, as well as improvement workshops
- Coach leaders and clinical teams on improvement and project methodologies
- Support the selection of key priorities towards a seamless patient experience
Qualifications:
- Undergraduate degree in nursing, allied health, business, health informatics or related healthcare discipline required
- Master’s degree in public administration, business administration, or healthcare administration or equivalent experience would be an asset
- Minimum 5 years of experience in project management or leading complex initiatives in healthcare environments such as hospitals, government or private sector required
- Experience in hospital work environment with complex flow projects would be an asset
- Experience with Lean, Six Sigma or other forms of process redesign preferred
- Superior analytical and problem solving abilities
- Demonstrated effectiveness at leading and working with teams
- Effective interpersonal communications skills and ability to successfully build relationships
- Strong working knowledge of computer applications, especially the Microsoft Office suite Satisfactory performance and attendance required
